大株红景天注射液联合曲美他嗪片治疗冠心病心力衰竭的临床疗效

Clinical Efficacy of Sofren Injection Combined with Trimetazidine Tablets in Patients with Coronary Heart Disease Complicated with Heart Failure

目的:探讨大株红景天注射液联合曲美他嗪片治疗冠心病心力衰竭的疗效及对患者N端-前脑利钠肽(NT-Pro BNP)、内皮素-1(ET-1)、去甲肾上腺素(NE)的影响。方法:选取2016年1月-2017年1月我院收治的84例冠心病心力衰竭患者为研究对象,按随机数字表法分为对照组与治疗组,各42例。对照组给予口服盐酸曲美他嗪片(20 mg,tid)治疗,治疗组在对照组基础上静滴大株红景天注射液(10 m L,qd)。治疗4周后,比较两组临床疗效、治疗前后左心室射血分数(LVEF)、左心室舒张末期内径(LVEDD)、左心房舒张末期内径(LAEDD)、NT-Pro BNP、ET-1、NE水平。结果:治疗后,治疗组总有效率为95.24%,高于对照组的76.19%,差异有统计学意义(P<0.05);治疗组LVEF水平高于对照组,差异有统计学意义(P<0.05),治疗组LVEDD、LAEDD、NT-Pro BNP、ET-1、NE水平均低于对照组,差异有统计学意义(P<0.05);两组不良反应比较差异无统计学意义(P>0.05)。结论:大株红景天注射液联合曲美他嗪治疗冠心病心力衰竭,能显著改善患者的心功能,降低NT-Pro BNP、ET-1、NE水平,临床效果显著,安全性较好,值得推广应用。

Objective:To investigate the efficacy of sofren injection combined with trimetazidine tablets in patients with coronary heart disease(CHD) complicated with heart failure and effect on N-terminal pro-brain natriuretic peptide(NT-Pro BNP), endothelin-1(ET-1) and norepinephrine(NE) in these patients. Methods:84 patients with CHD complicated with heart failure admitted to our hospital from January 2016 to January 2017 were selected as subjects investigated and randomly divided into control group(42 cases) and treatment group(42 cases). The patients in the control group were orally given trimetazidine dihydrochloride tablets(20 mg, tid), and the patients in the treatment group were given sofren injection by intravenous drip(10 m L, qd) on the basis of the control group. After 4 weeks of treatment, the clinical efficacy of the two groups was observed, and left ventricular ejection fraction(LVEF), left ventricular end-diastolic diameter(LVEDD), left atrial end-diastolic dimension(LAEDD), NT-Pro BNP, ET-1 and NE levels before and after treatment were compared between the two groups. Results:After treatment, the total effective rate of the treatment group was 95.24%, which was significantly higher than that(76.19%) of the control group(P<0.05); the level of LVEF in the treatment group was significantly higher than that in the control group(P<0.05); the levels of LVEDD, LAEDD, NT-Pro BNP, ET-1 and NE in the treatment group were significantly lower than those in the control group(P<0.05); there was no significant difference in the incidence of adverse reaction between the two groups(P>0.05). Conclusion:Sofren injection combined with trimetazidine can significantly improve the heart functions of the patients with CHD complicated with heart failure and reduce the levels of NT-Pro BNP, ET-1 and NE, so it has good clinical effect and good safety, and is worthy of promotion and application.
