2016年门诊中药饮片处方点评与分析

Review and Analysis of Prescriptions of Traditional Chinese Medicine Decoction Pieces at Outpatient Department in 2016

目的:对2016年我院门诊中药饮片处方进行点评,分析不合理处方存在的问题并提出改进措施。方法:随机抽取我院2016年1-12月每月50张门诊中药房中药饮片处方,共计600张。根据调查目的以及相关点评标准和规范,采用医院药品管理系统进行数据分类汇总,填写处方基本指标点评表以及不合理处理统计表,对抽取的所有处方进行点评。结果:600张门诊中药饮片处方中,男性326张,女性274张,年龄1~79岁,平均年龄(48.7±6.3)岁;单张处方用药3~35味,药物总剂量为36~360 g;其中不合理处方289张,占总抽查处方的48.2%,以处方书写不规范、用法用量不适宜和疾病诊断与用药不符最为常见。结论:处方点评是了解临床用药情况和提高医院合理用药水平的重要手段,进行高质量的中药饮片处方点评是医院管理中不可或缺的重要内容。

Objective:To review prescriptions of traditional Chinese medicine decoction pieces at outpatient department in 2016, analyze existing problems in unreasonable prescriptions and put forward improvement measures. Methods:50 prescriptions of traditional Chinese medicine decoction pieces per month from traditional Chinese medicine pharmacy of outpatient department in our hospital from January 2016 to December 2016 were randomly selected, a total of 600. According to investigation purposes, relevant review criteria and standards, the hospital drug management system was used to carry out data classification and summary, fill in review form of basic indexes of prescriptions and statistical form of unreasonable treatment and review all prescriptions drawn. Results:In above 600 prescriptions of traditional Chinese medicine decoction pieces at the outpatient department, the data of the prescriptions included 326 prescriptions for males and 274 prescriptions for females, age from 1~79 years, average age of(48.7±6.3) years; 3~35 medicines for single prescription and total dose of 36~360 g; 289 unreasonable prescriptions, accounting for 48.2% of the total chosen prescriptions, especially nonstandard prescription writing, inappropriate usage and dosage and discrepancies between disease diagnosis and drug use. Conclusion:The review of prescriptions are an important means to understand the status of clinical drug use and improve the level of rational drug use in hospitals, so the review of high-quality prescriptions for prescriptions of traditional Chinese medicine decoction pieces is an indispensable part of hospital management.
