宫腔镜下子宫纵隔切除术治疗子宫纵隔畸形不孕的疗效观察

Observation on Efficacy of Hysteroscopic Uterine Mediastinal Resection for the Treatment of Infertility Caused by Uterine Mediastinal Malformation

目的 :比较研究宫腔镜和经腹子宫纵隔切除术治疗子宫纵隔畸形不孕的效果,并近期随访观察术后对妊娠及妊娠结局的影响。方法:选取2015年1月—2016年6月我院收治的子宫纵隔畸形不孕患者80例,各40例。对照组给予经腹子宫纵隔切除术治疗,观察组给予宫腔镜下子宫纵隔切除术治疗。比较两组手术时间、术中出血量、住院时间,以及术后妊娠率、自然流产率、活产率。结果 :观察组手术时间、住院时间短于对照组,术中出血量少于对照组,差异有统计学意义(P<0.05);术后随访1年,观察组妊娠率、活产率高于对照组,自然流产率低于对照组,差异有统计学意义(P<0.05)。结论 :宫腔镜下子宫纵隔切除术治疗子宫纵隔畸形不孕可缩短手术时间、住院时间,减少术中出血,提高妊娠率,减少自然流产。

Objective: To compare the effect of hysteroscopy and transabdominal uterine mediastinal resection for the treatment of infertility caused by uterine mediastinal malformation, and to observe the effect on pregnancy and pregnancy outcomes after operation by follow-up. Methods: 80 cases of patients with infertility caused by uterine mediastinal malformation admitted to our hospital from January 2015 to June 2016 were selected and divided into two groups according to different operation methods, 40 cases in each group. The patients in the control group were given transabdominal uterine mediastinal resection, and the patients in the observation group were given hysteroscopic uterine mediastinal resection. The operation time, intraoperative bleeding amount, hospitalization time, postoperative pregnancy rate, spontaneous abortion rate and live birth rate were compared between the two groups. Results: The operation time and hospitalization time in the observation group were shorter than those in the control group and the intraoperative bleeding amount was less than that in the control group, and the difference was statistically significant(P<0.05). After one year of follow-up, the pregnancy rate and live birth rate in the observation group were higher than those in the control group and the spontaneous abortion rate was lower than that in the control group, and the difference was statistically significant(P<0.05). Conclusion: Hysteroscopic uterine mediastinal resection for the treatment of infertility caused by uterine mediastinal malformation can shorten operation time and hospitalization time, reduce intraoperative bleeding amount, improve pregnancy rate and reduce spontaneous abortion.
