单纯骶管注射曲安奈德联合药物理疗治疗腰椎间盘突出症的疗效比较

Comparison of Efficacy of Simple Sacral Canal Injection of Triamcinolone Acetonide Combined with Traditional Chinese Medicine in the Treatment of Lumbar Disc Herniation

目的:观察比较单纯骶管注射曲安奈德联合中药涂擦烫熨治疗腰椎间盘突出症(LDH)的临床疗效。方法 :选取2015年5月—2017年1月在我院治疗的60例腰椎间盘突出症患者,按随机数字表法分为两组,各30例。对照组(A组)给予骶管注射曲安奈德(40 mg)治疗,试验组(B组)在对照组基础上联合中药涂擦烫熨疗法治疗。比较两组治疗前后视觉模拟评分(VAS)、日本骨科学会评估治疗分数(JOA)和Oswestry功能障碍指数问卷表(ODI)评分。结果:两组治疗1周、2周、3周VAS评分、JOA评分和ODI评分较治疗前均明显改善,且B组优于A组,差异有统计学意义(P<0.05);两组治疗后3个月VAS评分和ODI评分较治疗前明显降低,JOA评分明显升高(P<0.05),但组间比较差异无统计学意义(P>0.05)。结论 :运用骶管注射曲安奈德配合中药涂擦烫熨治疗LDH临床疗效优于单纯使用骶管注射曲安奈德疗法,值得临床推广应用。

Objective: To observe the clinical efficacy of simple sacral canal injection of triamcinolone acetonide combined with Chinese medicine rubbing and ironing in the treatment of lumbar disc herniation(LDH). Methods: 60 patients with lumbar disc herniation admitted to our hospital from May 2015 to January 2017 were randomly divided into two groups, 30 cases in each group. The patients in control group(group A) were given sacral canal injection of triamcinolone acetonide(40 mg), and the patients in experimental group(group B) were given Chinese medicine rubbing and ironing on the basis of the control group. Visual analogue scale(VAS), Japanese orthopedics assessment(JOA) scores and Oswestry dysfunction index questionnaire(ODI) scores were compared between the two groups. Results: The VAS scores, JOA scores and ODI scores in the two groups after 1 week, 2 weeks and 3 weeks of treatment were significantly improved compared with those before treatment, and the group B was significantly better than the group A(P<0.05); the VAS scores and ODI scores in the two groups after 3 months of treatment were significantly decreased compared with those before treatment, while JOA scores were significantly increased(P<0.05), but there was no statistical difference between two groups. Conclusion: Sacral canal injection of triamcinolone acetonide combined with Chinese medicine rubbing and ironing is better than simple sacral canal injection of triamcinolone acetonide in the treatment of LDH, which is worthy of clinical generalization and application.
