Comparative Analysis of Outpatient Prescriptions Before and After Intervention in a General Hospital

目的:分析焦作市第二人民医院(以下简称"我院")门诊处方干预效果,探讨不合理处方干预方法,提高临床用药安全。方法:随机抽取我院2016年1月—12月(干预前)门诊处方及2017年1月—12月(干预后)门诊处方各18240张,比较处方不合理应用的指标变化,检验干预效果。结果 :干预前后抗菌药物使用率、注射剂使用率、基本药物使用率比较,差异有统计学意义(P <0.01);处方合格率由干预前的82.59%上升至干预后的93.85%;干预后不规范处方及不适宜处方较干预前明显减少,差异有统计学意义(P <0.01)。结论 :采取干预措施后,门诊处方大部分指标有显著改善,但仍有指标未达到规定要求,还存在不合理用药现象。应不断对医师处方实施干预,促进临床合理用药。

Objective:To analyze the intervention effect on outpatient prescriptions in the Second People’s Hospital of Jiaozuo City(hereinafter referred to as our hospital)to explore the irrational prescription intervention methods and to improve the safety of clinical drug use.Methods:18240 outpatient prescriptions from January to December 2016(before intervention)and from January to December 2017(after intervention)in our hospital were randomly sampled.The changes in indexes of the irrational use of prescriptions were compared and the effect of intervention was tested.Results:There were statistically significant differences in antibiotics use rate,injection use rate and basic drug use rate before and after intervention(P<0.01).The qualified rate of prescriptions increased to 93.85%after intervention from 82.59%before intervention.The irregular prescriptions and unsuitable prescriptions after intervention significantly decreased compared with those before intervention,and the differences were statistically significant(P<0.01).Conclusion:After the intervention,most of the indexes of outpatient prescriptions have improved significantly,but there are still indexes that do not met the prescribed requirements,and there are still irrational drug use.Interventions should be continuously implemented on physicians’prescriptions to promote clinical rational drug use.
