低分子肝素辅助治疗小儿重症肺炎对患儿潮气呼吸肺功能及血清PCT、乳酸水平的影响分析 被引量:6

摘要 目的:研究低分子肝素辅助治疗小儿重症肺炎及对患儿潮气呼吸肺功能、血清降钙素原(PCT)、乳酸水平的影响。方法 :选取我院收治的146例重症肺炎患儿,按随机数字表法分为两组,各73例。对照组采用常规治疗,观察组基础上加用低分子肝素[1 mg/(kg·d),分4次静脉注射]。比较两组疗效及治疗前后潮气呼吸肺功能、血清降钙素原(PCT)、乳酸水平。结果 :观察组治疗有效率高于对照组,差异有统计学意义(P <0.05);治疗后,两组呼吸频率(RR)和呼气峰值流速(PEF)较治疗前明显降低,且观察组低于对照组,差异有统计学意义(P <0.05);单位体质量潮气量(TV/kg)、吸气与呼气时间比值(TI/TE)、达峰时间比(TPTEF/TE)和达峰容积比(VPEF/VE)较治疗前明显升高,且观察组高于对照组,差异有统计学意义(P <0.05);治疗后,两组血清PCT和乳酸水平较治疗前明显降低,且观察组低于对照组,差异有统计学意义(P <0.05);两组治疗过程中均未见明显不良反应发生。结论 :低分子肝素辅助治疗小儿重症肺炎的疗效显著,可有效改善患儿肺功能,降低血清PCT及乳酸表达水平,且安全性高。 Objective:To study the effect of low molecular weight heparin in the adjunctive treatment of pediatric severe pneumonia on tidal respiratory lung function and serum procalcitonin(PCT)and lactic acid levels.Methods:146 children with severe pneumonia admitted to our hospital were selected and randomly divided into two groups,73 cases each.The patients in the control group were given conventional treatment,and the patients in the observation group were further treated with low molecular weight heparin(1 mg/(kg·d),intravenous injection 4 times)The efficacy,tidal respiratory lung function,serum procalcitonin(PCT)and lactic acid levels before and after treatment were compared between the two groups.Results:The effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05).The respiratory rate(RR)and peak expiratory flow rate(PEF)in the two groups after treatment were significantly lower than those before treatment,and the RR and PEF in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).The tidal volume per kg unit body mass(TV/kg),inspiratory and expiratory time ratio(TI/TE),peak time ratio(TPTEF/TE)and peak volume ratio(VPEF/VE)after treatment were significantly higher than those before treatment,and these indexes in the observation group were higher than those in the control group,with statistically significant differences(P<0.05).The levels of serum PCT and lactic acid after treatment were significantly lower than those before treatment,and the levels in the observation group were lower than those in the control group,with statistically significant differences(P<0.05).There were no significant adverse reactions during the treatment in the two groups.Conclusion:Low molecular weight heparin is significantly effective in the treatment of pediatric severe pneumonia,which can effectively improve lung function and reduce serum PCT and lactic acid expression levels,and has high safety.
