低分子肝素钙联合压力抗栓泵预防髋关节置换术后下肢深静脉血栓(DVT)形成的效果观察

Observation on Preventive Effect of Low Molecular Weight Heparin Calcium Combined with Pressure Antithrombotic Pumps on Deep Venous Thrombosis (DVT) Formation after Hip Arthroplasty

目的:分析观察低分子肝素钙联合压力抗栓泵预防髋关节置换术后下肢深静脉血栓(DVT)形成的效果。方法 :选取2015年10月—2016年10月在我院接受髋关节置换术的80例患者为研究对象,按随机数字表法分为对照组和试验组,各40例,对照组给予基础预防,试验组给予低分子肝素钙(术后12 h给予2 500 IU,脐周皮下注射,术后第一日开始给予5 000 IU,qd,脐周皮下注射)结合压力抗栓泵预防治疗,比较两组DVT发生情况及治疗前后深静脉血流速度和下肢周径。结果 :试验组DVT发生率低于对照组,差异有统计学意义(P <0.05);治疗后试验组深静脉血流速度高于对照组,下肢周径小于对照组,差异有统计学意义(P <0.05)。结论 :低分子肝素钙结合压力抗栓泵预防髋关节置换术后DVT发生的效果明显,值得临床推广应用。

Objective:To observe the resisting effect of low molecular weight heparin calcium combined with pressure antithrombotic pumps on deep venous thrombosis(DVT)after hip arthroplasty.Methods:80 patients who underwent hip arthroplasty in our hospital from October 2015 to October 2016 were selected as subjects investigated and randomly divided into a control group and an experimental group,40 cases each.The patients in the control group were given basic prevention,and the patients in the experimental group were given low molecular weight heparin calcium(2 500 IU given at 12 h after surgery,subcutaneous injection,5 000 IU given on the first day after surgery,qd,periumbilical subcutaneous injection)combined with pressure antithrombotic pumps for preventive treatment.The incidence of DVT,deep venous blood flow velocity and limb extremity circumference before and after treatment were compared between the two groups.Results:The incidence of DVT in the experimental group was lower than that in the control group,and the difference was statistically significant(P<0.05).After treatment,the deep venous blood flow velocity in the experimental group was higher than that in the control group,and the lower extremity circumference was smaller than that in the control group,with statistically significant differences(P<0.05).Conclusion:Low molecular weight heparin calcium combined with pressure antithrombotic pumps is effective in preventing DVT after hip arthroplasty,which is worthy of clinical application.
