氨溴索联合异丙托溴铵对胸腔镜肺叶切除术患者肺功能及炎症反应的影响

Effect of Ambroxol Combined with Ipratropium Bromide on Pulmonary Functions and Inflammatory Response in Patients Undergoing Thoracoscopic Lobectomy

目的:观察应用氨溴索与异丙托溴铵联合治疗胸腔镜肺叶切除术的效果。方法:选取2018年1月—2018年12月于我院采用胸腔镜肺叶切除术治疗的肺癌患者68例,按照随机数字表法分为观察组与对照组,各34例。对照组给予0.9%氯化钠注射液,观察组采用氨溴索(150 mg+100 mL 0.9%氯化钠注射液,静脉滴注,bid)与异丙托溴铵(6 mL+2 mL 0.9%氯化钠注射液,雾化吸入,bid)联合治疗。比较两组肺功能[1s用力呼气容积(FEVl)、最大呼气峰流速(PEF)、用力肺活量(FVC)]、炎症反应(痰白细胞计数、C反应蛋白)。结果:两组术后FEVl、PEF以及FVC值较术前均下降,但观察组高于对照组,差异有统计学意义(P <0.05);观察组术后1 d、7 d时痰白细胞计数及C反应蛋白均低于对照组,差异有统计学意义(P <0.05)。结论:针对胸腔镜肺叶切除手术治疗的肺癌患者,予以异丙托溴铵与氨溴索联合治疗可有效改善肺功能,缓解炎症反应,改善疾病转归。

Objective:To observe the effect of ambroxol combined with ipratropium bromide on thoracoscopic lobectomy.Methods:68 patients with lung cancer who underwent thoracoscopic lobectomy in our hospital from January 2018 to December 2018 were selected and randomly divided into an observation group and a control group,34 cases each.The patients in the control group were 0.9%sodium chloride injection,and the patients in the observation group were treated with ambroxol(150 mg+100 mL 0.9%sodium chloride injection,intravenous drip,bid)combined with ipratropium bromide(6 mL+2 mL 0.9%sodium chloride injection,aerosol inhalation,bid).The pulmonary functions[forced expiratory volume in one second(FEV l),maximum peak expiratory?ow(PEF),forced vital capacity(FVC)]and in?ammatory response(sputum white blood cell count,C-reactive protein)were compared between the two groups.Results:The values of FEV l,PEF and FVC in the two groups after operation were lower than those before operation,but the values in the observation group were higher than those in the control group,with statistically signi?cant differences(P<0.05).The sputum white blood cell count and C-reactive protein 1 day and 7 day after operation in the observation group were lower than those in the control group,with statistically signi?cant differences(P<0.05).Conclusion:For the patients with lung cancer treated by thoracoscopic lobectomy,ipratropium bromide combined with ambroxol can effectively improve pulmonary functions,relieve in?ammatory response and improve disease outcome.
