盐酸纳美芬对颅脑损伤患者术后脑血流动力学及认知功能的影响 被引量:4

Effect of Nalmefene Hydrochloride on Cerebral Hemodynamics and Cognitive Function in Patients with Craniocerebral Injury

摘要 目的:分析盐酸纳美芬对颅脑损伤患者术后脑血流动力学及认知功能的影响。方法:选取2017年2月—2019年1月本院收治的颅脑损伤患者80例,以随机数字表法分为两组,各40例。两组在入组前均终止其他治疗,并均予以常规治疗,对照组予以亚低温治疗,观察组在对照组基础上予以盐酸纳美芬[0.7 mg/(kg·h),bid,]治疗。比较两组脑血流动力学指标、血清相关指标及认知功能。结果:观察组治疗后大脑双侧动脉收缩期峰值血流速度(Vs)、平均血流速度(Vm)、简易精神状态检查(MMSE)评分均高于对照组,搏动指数(PI)、S100-β、神经胶质纤维酸性蛋白(GFAP)水平低于对照组,差异有统计学意义(P <0.05)。结论:盐酸纳洛美用于治疗颅脑损伤可有效改善患者的脑血流动力学指标与认知功能,减轻脑神经损伤。 Objective:To analyze the effect of nalmefene hydrochloride on cerebral hemodynamics and cognitive function in the patients with craniocerebral injury.Methods:80 patients with craniocerebral injury admitted to our hospital from February 2017 to January 2019 were selected and randomly divided into two groups,40 cases each.The patients in the two groups terminated other treatments before admission,and were given conventional treatment.The patients in the control group were further treated with mild hypothermia,and the patients in the observation group were given nalmefene hydrochloride[0.7 mg/(kg.h),bid]on the basis of the control group.The cerebral hemodynamic indexes,serum correlative indexes and cognitive function were compared between the two groups.Results:The peak systolic blood?ow velocity(Vs),mean blood?ow velocity(Vm)and mini-mental state examination(MMSE)scores in bilateral cerebral arteries in the observation group were higher than those in the control group,while the levels of pulsatility index(PI),S100-β,glial?brillary acidic protein(GFAP)were lower than those in the control group,and the differences were statistically signi?cant(P<0.05).Conclusion:Nalmefene hydrochloride for the treatment of craniocerebral injury can effectively improve cerebral hemodynamic indexes and cognitive function of the patients and reduce cranial nerve injury.
