脑梗死后代谢产物演变的磁共振化学位移成像研究 被引量:8

摘要 目的:采用化学位移成像(CSI)研究脑梗死区代谢产物的演变,分析梗死不同区域间代谢产物浓度变化的差异。材料和方法:完成57例次非腔隙性脑梗死CSI检查,在不同时期及不同部位(梗死核心、内缘、外缘及正常)比较各代谢产物相对浓度。结果:(1)梗死区NAA持续下降;Lac上升迅速,急性期以后开始下降;Cr、Cho在亚急性期才较明显下降,慢性期Cho略回升,以边缘较明显。(2)NAA下降与DWI所示病变范围一致;Lac上升于急性期可超出该范围,亚急性期这一现象消失。结论:CSI反映出脑梗死灶不同区域之间物质浓度的差异;急性期Lac升高范围超过NAA下降区域,可能预示着缺血半影区。 Purpose: To evaluate the evolution of metabolites in cerebral infarction,and to analyze the concentration changes of metabolites among different lesion areas in each stage by chemical shift imaging(CSI).Materials and Methods: CSIs were performed on 57 patients with none lacunar infarction.The relative concentrations were compared in different areas(core,inner rim,outer rim and normal) and stages.Results: (1) NAA of the lesions decreased continuously.Lac increased obviously and began to fall down after acute stage.Cr and Cho decreased in subacute stage and then Cho rose slightly in the rim during chronic stage.(2) The area with NAA decreased was accordant well with the lesion showed by DWI.The area with Lac increased was larger in acute stage but not in subacute stage.Conclusion: CSI can reflect the distribution of metabolites in cerebral infarction.In acute stage,the region with Lac increased was larger than that NAA decreased,which may indicate penumbra.
