摘要 目的探讨多层螺旋CT血管成像(MSCTA)对椎动脉起源变异与走行变异的相关性及临床意义。资料与方法回顾性分析114例椎动脉起源变异和(或)走行变异患者的MSCTA资料,应用容积再现、最大密度投影、曲面重组和高级血管分析等后处理技术进行处理,分析椎动脉起源变异和走行变异的相关性。结果114例中,67例仅走行变异,2例仅起源变异,45例同时具有起源变异和走行变异。起源变异与走行变异的差异有统计学意义(χ2=235.67,P<0.05),比值比为194.44,95%CI为46.13~819.67。结论椎动脉起源变异与走行变异间存在相关性,起源变异是导致走行变异的重要因素。MSCTA可以清晰客观地显示椎动脉的起源变异和走行变异,对了解椎动脉变异的影像学表现及治疗方案的制订有重要价值。 Purpose To evaluate the clinical value of multi-slice spiral CT angiography(MSCTA) in evaluating the correlation between origin variation and path variation of the vertebral artery. Materials and Methods MSCTA features of 114 patients with vertebral artery origin and(or) path variation were evaluated retrospectively, post-process techniques such as volume rendering(VR), maximum intensity projection(MIP), curve plane reformation(CPR) and advanced vessel analysis(AVA) were used to observe the vertebral artery, and the correlation between the origin variation and path variation of vertebral artery were analyzed. Results Among the 114 cases, there were 67 cases with only origin variation and 2 with only path variation of the vertebral artery, and 45 cases with both origin variation and path variation. The origin variation and path variation had statistical difference(χ2=235.67, P<0.05), with odds ratio and 95% CI of 194.44 and 46.13-819.67, respectively. Conclusion The origin variation and path variation of vertebral artery are well related, with the origin variation an important influencing factor leading to path variation. MSCTA is able to demonstrate the origin variation and path variation of the vertebral artery clearly and objectively, thus has important application value in understanding the imaging features of anatomic variations of vertebral artery and directing clinical treatment protocols.
