远红外热成像在缺血性脑血管病中的应用 被引量:11

摘要 目的分析远红外热成像对缺血性脑血管病的诊断效果,并探讨其应用价值。资料与方法回顾性分析103例缺血性脑血管病患者,作为观察组;39例健康体检者,作为对照组;采用ATIR-M301B非制冷焦平面医用远红外热像仪,采集头面部红外热像图,分析不同检测区域的温度差,比较两者结果;与数字减影血管造影结果进行对比;并分析经皮血管内支架成型术前后红外热像图变化特征。结果观察组眶上区、内眦区、额区、面颊区对称区域温度差值明显大于对照组,差异有统计学意义(t=2.335、1.062、1.536、2.627,P<0.05);32例治疗前后热像图温度改变,眶上区、内眦区、额区、面颊区温度差值减小,差异有统计学意义(t=2.352、0.873、1.469、0.665,P<0.05)。红外热像图诊断缺血性脑血管病的准确率为83%。结论远红外热像图可以作为缺血性脑血管病,尤其是颈内系统病变诊断的无创的功能影像客观评价指标,具有一定的临床应用价值。 Purpose To explore the value of far-infrared thermography in the diagnosis of patients with ischemic cerebrovascular diseases. Materials and Methods A retrospective study of 103 patients with ischemic cerebrovascular diseases as observation group and 39 normal healthy individuals as control group was carried out. We applied ATIR-M301B uncooled medical infrared camera to collect craniofacial infrared images, analyzed temperature differences among different detection zones and compared the results; we also compared the results of thermography with those obtained by digital subtraction angiography and analyzed the consistency; moreover, we analyzed infrared image characteristics before and after percutaneous endovascular stent angioplasty. Results Compared with those in control group, temperature differences in bilaterally symmetrical parts of supraorbital region, endocanthion, frontal region, buccal region were significantly higher in observation group(differences with statistic significance, t=2.335, 1.062, 1.536, 2.627; P<0.05); the craniofacial temperatures of 32 patients changed after operation, and the differences reduced in supraorbital region, endocanthion, frontal region, buccal region(differences with statistic significance, t=2.352, 0.873, 1.469, 0.665; P<0.05). Compared with digital subtraction angiography, the overall diagnostic accuracy of thermography was 83%. Conclusion The far-infrared thermography has clinical value in evaluating ischemic cerebrovascular disease, especially in patients with lesions of the internal carotid system, because it could provide objective noninvasive functional imaging.
