温阳益心法治疗冠心病冠状动脉狭窄32例临床疗效的回顾性研究

A Retrospective Study on the Clinical Efficacy of Wenyang Yixin Therapy(温阳益心法) in the Treatment of 32 Cases of Coronary Artery Stenosis

目的评价温阳益心法治疗冠状动脉狭窄的临床疗效。方法采用资料收集、电话回访的方式,对2010年5月至2018年5月连续就诊于中国中医科学院中医门诊部,经冠状动脉造影或冠状动脉CT确诊的冠心病患者32例进行回顾性研究。对温阳益心法治疗前后冠状动脉狭窄程度、临床症状评分、心绞痛及心电图疗效、硝酸甘油减停率、血压、血脂、血糖及安全性进行评定。结果 32例患者平均接受温阳益心法治疗(15. 97±10. 17)个月。治疗后冠状动脉狭窄程度总积分减少(13. 17±15. 81)分,较治疗前差异有统计学意义(P <0. 05)。32例患者共100处狭窄,治疗后56. 58%的中度以上狭窄减少或消失,狭窄度减少(16. 45±31. 25)%,较治疗前差异有统计学意义(P <0. 01)。治疗后心前痛、胸闷等症状基本消失(P <0. 01),症状总有效率84. 38%,心电图改善率73. 33%,硝酸甘油停减率94. 74%。治疗后收缩压、低密度脂蛋白胆固醇、低密度脂蛋白胆固醇/高密度脂蛋白胆固醇水平降低(P <0. 05),期间未见肝肾功能损伤不良反应及心血管事件发生。结论温阳益心法可有效减轻冠心病冠状动脉狭窄患者狭窄程度,特别是中重度狭窄,同时显著减轻患者临床症状、改善心肌缺血、缓解心绞痛,且无明显毒副作用。

Objective To evaluate the clinical effects of Wenyang Yixin Therapy(温阳益心法)on coronary artery stenosis.Methods A retrospective study was conducted on 32 patients with coronary heart disease who were diagnosed by coronary angiography or coronary CT in the Chinese Medicine Clinic of China Academy of Chinese Medical Sciences from May 2010 to May 2018,using data collection and telephone interviews.The degree of coronary artery stenosis,clinical symptom score,angina pectoris and electrocardiogram,nitroglycerin reduction rate,blood pressure,blood lipids,blood glucose and safety were evaluated before and after treatment with Wenyang Yixin Therapy.Results Thirty-two patients received Wenyang Yixin Therapy for 15.97±10.17 months on average.After treatment,the score of coronary stenosis decreased by 13.17±15.81,and the difference was statistically significant(P<0.05).A total of 100 plaques occurred in 32 patients,and 56.58%of moderate to severe plaques disappeared or decreased after treatment,and the stenosis degree decreased by(16.45±31.25)%.The difference was statistically significant(P<0.01).Symptoms such as chest pain and chest congestion basically disappeared(P<0.01),with effective rate as 84.38%.The electrocardiogram effective rate was 73.33%,and the reduction rate of nitroglycerin was 94.74%.After treatment,the levels of systolic blood pressure,low density lipoprotein cholesterin(LDL-C),ratio of LDL-C to high density lipoprotein cholesterin(HDL-C)were decreased(P<0.05).No liver and renal damage,adverse reactions or cardiovascular events were observed during treatment.Conclusion Wenyang Yixin Therapy can effectively reduce the degree of coronary stenosis in patients with coronary heart disease,while significantly alleviate patients’clinical symptoms,improve myocardial ischemia and relieve angina,without significant toxin and adverse effects.
