刘沈林抑木扶土法治疗胃癌术后经验

LIU Shenlin’s Experience in Treating Postoperative Gastric Cancer with Inhibiting Wood and Strengthening Earth Method

刘沈林教授认为,胃癌术后多见肝脾不调证,一方面由于肝郁气滞,气不行津运血而致痰瘀渐聚;另一方面脾虚失运,虚体易受邪侵,并且脾虚与肝郁相互影响。针对胃癌术后患者肝脾不调的证候特点,采用抑木扶土法恢复中焦平衡,治标从气郁入手重在调肝,治本重在健脾以扶正。在选用健脾药时,喜用气阴双补、柔和滋润之品,治肝时常以理气与活血、消癥、散结之品配伍同用,从而保持中焦升降有序,预防复发转移。

Professor LIU Shenlin believes that syndrome of unregulated liver and spleen is frequently seen after surgery for gastric cancer.On the one hand,due to liver qi stagnation,qi cannot move fluid and transport blood,and then cause accumulation of phlegm and stasis;on the other hand,spleen deficiency fails to transport,and deficient body is vulnerable to evil invasion.The spleen deficiency and liver depression affect each other.In view of the characteristics of the syndrome of unregulated liver and spleen in patients with postoperative gastric cancer,the balance of the middle jiao was restored by using the method of inhibiting wood and strengthening earth.The treatment of the symptoms was from qi stagnation,and focused on regulating the liver.The treatment of root causes focused on invigorating the spleen to reinforce the vital qi.When choosing the medicinals of invigorating the spleen,the herbs of tonifying qi and yin,soft and moisturizing were preferred.When treating the liver,herbs of regulating qi is often used in combination with the herbs of activating blood,eliminating symptoms and dissipating the mass,so as to keep orderly ascending and descending of the middle jiao and prevent recurrence and metastasis.
