肝癌患者5-氟尿嘧啶血药浓度的监测与测定方法的改进 被引量:5

摘要 目的 :本文用改进的高效液相色谱 (HPLC)法测定 8位肝癌患者静脉滴注 750mg 5 Fu后 7个时间点的血药浓度。方法 :采用HPLC 紫外检测 (标准曲线法 )。提取 :改用乙腈加异丙醇 ;流动相 :甲醇∶乙腈∶水 (2 5∶5∶70 ) ,内加四丁基溴化铵 ,检测波长 2 65nm。结果 :分离良好 ,完全无血杂质干扰 ;回收率高 (>91 % ) ,日内精密度、日间精密度 <6 % ,监测结果血浓个体差异极大 (2 .5倍以上 )。结论 :本法取血量少 ,提取方法简单 ,离子对试剂效果佳 ,分离良好 ,灵敏度高 ,线性范围宽 (0 .2~2 5 .0mg·L- 1 ) ,适用于基层医院的治疗药物监测 (TDM)。本实验证实 ,5 Fu血浓个体差异较大 ,应该进行TDM ,以实行个体化给药方案 ,使用药更安全、有效、合理和经济。 OBJECTIVE To determine the concentration of 5-Fu in serum at seven time points from 8 Hepatic Cancer patients an by improved method of HPLC. METHODS HPLC-UV Determination was employed.Acetonitrile and iso-propanol were used as the extractive solvent.The mobile phase was methanol-acetonitrile-water(25∶5∶70), tetrabutylamino∶bromide was added.The wavelength for determination was 265 nm.RESULTS Good seperation and high recoveries (>91%) were obtained, without any interference from impurities of serum.Coefficients of variation within a day and between days were less than 6%. The result indicated: individual difference of the drug concentration was significant. CONCLUSIONS Less blood sample has been used by using this method.Extraction method was simple ,ion-pair agent has good spearation.and high sensitivity, good precision and wide liner range ( 0.2 ~ 25.0 mg·L -1 ).It was suitable to determine the concentration of 5-Fu in serum for basic level hospitals. The difference of the concentration of 5-Fu in patients' serum is rather significant.The TDM should be used for the individual administration,which will make the drug administration to be more safe, effective, reasonable and economical.
