颈动脉狭窄伴有同侧大脑中动脉狭窄血管内支架治疗一例 被引量：2

摘要 病例报告:患者男 ,56岁 ,左侧肢体无力频繁发作 ,内科给予纤维蛋白溶解、抗血小板治疗 2周后,症状仍然发作,每天平均 2～ 3次. 患者有高血压病史 10年,入院时血压 170/ 90mm Hg(1mm Hg=0.133kpa),入院后 CT平扫未见明显异常, MRI显示右侧底核区小腔隙性梗塞灶 (图 1), SPECT葡糖糖半定量显示右侧半球大脑中动脉供血区代谢减低 (图 2), MRI灌注成像显示局部脑血流量下降 (图 3),平均经过时间延长 (图 4).入院后通过常规检查后行血管造影检查,发现右侧颈内动脉起始段狭窄 70%,伴有溃疡形成 (图 5).右侧大脑中动脉 M1段高度狭窄 (70%,图 6),同侧大脑前动脉 A1段发育不好.经过讨论决定同时行颈内动脉起始段和同侧 M1段狭窄置于支架血管成形术.
