男性乳腺癌7例临床分析

Clinical analysis of 7 cases with male breast cancer

目的 :探讨男性乳腺癌的临床特征、治疗效果和预后情况。方法:对 1989～ 2 0 0 0年我院肿瘤外科收治的 7例男性乳腺癌的临床资料进行回顾性分析 ,并对男性乳腺癌的临床特点、病因、治疗效果和影响预后的因素进行了探讨。 7例中 4例行根治术 ,1例行局部切除 +腋窝淋巴结清扫术 ,2例行姑息性放疗。 5例 (74 .1% )腋下有转移者 ,术后 3例进行了正规辅助治疗。结果 :经过随访 ,3例 、 期患者 5年内无死亡 ,总的 5年生存率为 4 2 .9%(3/7)。 结论:治疗男性乳腺癌应以手术治疗为主 ,同时辅助放疗、化疗、内分泌等综合治疗 ;男性乳腺癌预后与临床分期、腋窝淋巴结状况有关 ;男性乳腺癌较少见且发病年龄大 ,临床病例晚期多见 ,淋巴结转移早 ,预后差 ,而年龄等因素对预后影响不大。

Objective: To study the clinical characteristics?treatment effect and prognosis of male breast cancer. Methods: 7 cases of male breast cancer were reviewed and followed up,along with domestic and oversea material,the clinical characteristics?pathogenesis?treatment effect and prognostic factors of male breast cancer were studied. All those patients who were diagnosed and treated in our hospital between 1989 and 2000. Of the 7 cases 4 cases accepted the radical operation,1 case accepted local operation and axillary lymph node operation. Positive for axillary lymph node were found in 5 cases (74.1%),3 cases accepted post-operation standard supplementary therapy. Results: All cases were followed up. Patients of stage Ⅰ,Ⅱ were alive during 5 years,overall 5-year survival rate was 42.9%(3/7). Conclusions: Male breast cancer is rare and the age of most patients is old. Advanced disease is common. Lymph node metastases takes place early. Prognosis is bad. Operation is a main treatment measure,meanwhile,assisted with systemic treatment,such as radiotherapy\,chemotherapy and endocrine therapy and so on. Prognosis corelates with the stage and axillary lymph node states,whereas it has a little relation to others factors,such as age.
