拉力螺丝钉配合髁支持钢板治疗股骨远端粉碎性骨折

Treatment of Distal Femoral Comminuted Fractures Utilized with Condylar Steel Plate Matched with Tensile Screw

目的 :总结拉力螺丝钉配合髁支持钢板治疗股骨远端粉碎骨折的临床效果。方法 :14例股骨远端粉碎性骨折采用髁支持钢板及拉力螺丝钉内固定 ,股骨髁间用 2～ 3枚松质骨螺丝做拉力固定 ,较小骨折块用拉力螺丝钉固定在较大的骨折块上。骨折按AO/ASIF分类 :A3型 1例 ,C1型 3例 ,C2型 7例 ,C3型 3例。结果 :平均随访 14个月 ,13例骨折正常愈合 ,1例延迟愈合 ,平均骨折愈合时间为 4个月 ;按Kolmenrt评定标准 :本组优 7例 ,良 5例 ,一般 1例 ,差 1例 ,优良率 85 .7%。膝关节活动度 5 5～ 12 0° ,平均膝关节活动度 98°。结论 :拉力螺丝钉配合髁支持钢板可有效治疗股骨远端粉碎性骨折 ,具有固定牢固、骨折愈合率高、并发症少等优点。

Objective: To summarize the treatment effect of distal femur comminuted fractures utilized with condylar steel plate matched with tensile screw.Method:Fourteen fractures were treated by condylar steel plate and tensile screw,intercondylar fractures were fixed by 2～3 screws,the small fracture pieces were fixed on the bigger fracture pieces.The fractures were classified according AO/ASIF:1 case of A3,3 cases of C1,7 cases of C2 and 3 cases of C3.Results: With averaged 14 months follow up,13 fractures healed,1 case healed delayed,and mean union time were 4 months.According to the standard of Kolmenrt:7 cases of excellent,5 cases of good,1 case of common,the rate of good was 85.7%.The average knee range of motion was 98 degrees.Conclusions:The condylar steel plate and tensile screw were effective for the treatment of the distal femoral fractures.
