开颅术后缄默症

Postoperative Mutism after Craniotomy

目的 探讨开颅术后缄默症的发病机理及预防和预后。方法 回顾性分析开颅术后缄默症 7例。结果 幕下手术 5例 ,幕上 2例 ,其中肿瘤 5例 ,外伤 2例 ,出现缄默症的时间为术后 1～ 7天 ,平均 2 .7天 ,持续时间 6～ 67天 ,平均 2 5 .4天。结论 开颅术后缄默症可能与手术、外伤、颅内感染等诸多因素致齿状核 -丘脑 -大脑皮质两侧言语通路中断有关。

Objectives To inquire into the mechanism, prevention and prognosis of postoperative mutism after Craniotomy. Methods 7 cases of postoperative mutism after craniotomy were analyzed retrospectively. Results 5 cases of infratentorial operation and 2 cases of supratentorial operation were performed, and 5 cases of them were tumor, 2 cases of them were trauma. The mutism was occurred at 1～7 days after operation, the average time was 2.7 days. The mutism lasted 6～67days, and 25.4 days on the average. Conclusions Postoperative mutism after craniotomy may be related to blocking of bilateral speech conduction of dentate nucleus-thalamus-cerebral cortex caused by operation, trauma, intracranial infection and other factors.
