草酸铂联合5-氟脲嘧啶与甲酰四氢叶酸治疗晚期大肠癌临床观察

A Clinical Study of Oxaliplatin Plus 5-FU/cf in Treatment of Advanced Colorectal Cancer

目的 观察草酸铂联合化疗治疗晚期大肠癌临床疗效。方法 3 2例晚期大肠癌患者 ,用草酸铂 13 0mg/m2 ,静脉滴注 2小时 ,化疗d1,甲酰四氢叶酸 10 0mg/m2 化疗d1～ 5 ;5 氟脲嘧啶 0 .5g静脉滴注化疗d1～ 5 ,每 3周重复一次 ,治疗 2周期后评定疗效 ,有效病例 4周期后确认疗效。结果 3 2例中CR 3例 ,PR 11例 ,SD 14例 ,PD 4例 ,总有效率 (CR +PR) 43 .8%。主要毒副作用 :Ⅰ～Ⅱ度恶心、呕吐 ,腹泻 ,感觉神经毒性 ,黏膜炎 ,骨髓抑制 ,静脉炎等。结论 草酸铂联合 5 FU +CF治疗晚期大肠癌效果良好 ,不良反应可耐受 ,值得临床推广。

Objective To evaluate the clinical efficacy and toxicity of Oxaliplatin plus 5-FU/CF combination in advanced colorectal cancer. Methods Thirty-two patients with advanced colorectal cancer were treated with Oxaliplatin plus 5-FU/CF. Oxaliplatin 130 mg/m 2 in infusion for two hours , in the first day and cf 100mg/m 2 in infusion ,from the first day to the fifth day ,and 5-FU 0.5g/m 2 in infusion, from the first day to the fifth day. The above-mentioned regimen was repeated every three weeks . The curative effect was evaluated two weeks later, and the response for the effective patients were confirmed four weeks later. Results Among the total 32 cases, there were CR 3,PR 11, SD 14, PD 4, with an overall response rate (CR+PR) of 43.8%. The main adverse effects were nausea and vomiting , diarrhea, neurousensory ,toxicity , mucositis, neutropenia and peripheral phlebitis. Conclusion Oxaliplatin is safe and effective in treating advanced colorectal cancer.The effect of 5-FU/CF is enhanced significantly when Oxaliplatin is combined. The toxicity is mild and tolerable for most of the patients.
