RT-PCR法检测乳腺癌前哨淋巴结微转移的研究 被引量:3

摘要 目的 探讨乳腺癌前哨淋巴结 (SLN)定位及其微转移检测的临床意义。方法 对 4 2例乳腺癌患者采用术中肿瘤周围和乳晕下注射专利蓝定位SLN ;用RT -PCR方法检测SLN中CK19的表达。结果 SLN定位成功率 88.1% (37 4 2 )。SLN的成功定位与患者的年龄有关。RT -PCR法与常规病理检查方法相比 ,检测SLN转移的阳性率提高 10 .2 % ,SLN预测腋窝淋巴结 (ALN)状态的精确性提高 5 .4 % ,二者具极显著性差异 (P <0 .0 0 1)。结论通过SLN定位和RT -PCR的联合应用 ,可明显提高乳腺癌SLN微转移的检出率 ,提高SLN预测ALN状态的准确性。 Objective To evaluate the clinical significance of locating sentinel lymph node (SLN) and detecting SLN micrometastases.Methods In forty-two breast cancer patients, Patent blue-Ⅴdye was injected into multiple sites around the tumor and the subareolars to locate the SLN, and RT-PCR was used to examine the expressions of CK19.Results The SLN was successfully identified in 88.1%(37/42) of the patients.It was related to age. Detection rate of SLN metastases increased 10.2% and the accuracy of the SLN in predicting the axillary lymph node (ALN) status increased 5.4% by applying RT-PCR method,There was a significant difference between RT-PCR method and routine pathological examination (P<0.001).Conclusion The combined utilization of locating SLN and RT-PCR can greatly enhance the efficiency of micrometastases detection in SLN, and increase the accuracy of the SLN in predicting ALN status in breast cancer. [
