新硅胶推注式人工晶状体与PMMA人工晶状体临床效果比较

The comparison of clinical effects of advanced generation injector-type silicone intraocular lens with PMMA intraocular lens

目的 探讨新一代硅胶推注式人工晶状体植入术的临床应用效果。方法 对白内障超声乳化吸除术 10 2眼行新一代硅胶推注式人工晶状体植入术 (A组 ) ,10 8眼行PMMA人工晶状体植入术 (B组 ) ;并对两组患者术后视力、术前术后角膜散光、术后前房闪辉、术后角膜水肿、人工晶状体位置、后囊混浊的变化及其它手术并发症进行观察。结果 术后 1周 ,裸眼视力≥ 0 5眼数为70 3 % (A组 )和 5 2 8% (B组 ) ;术后 1个月 ,球镜矫正视力≥ 1 0眼数为 71 8% (A组 )和5 5 4% (B组 ) ;术后 3个月 ,球镜矫正视力≥ 1 0眼数为 70 3 % (A组 )和 5 3 6% (B组 ) ,上述统计学处理均相差显著。术后 1个月与术前相比 ,角膜平均散光度数变化 ,A组无显著差异 ,B组有显著差异。术后前房闪辉、角膜水肿、后囊混浊、人工晶状体移位的发生率A组显著低于B组。A组人工晶状体在前房内的破损率和翻转率分别为 2 9%和 1 0 %。结论 新一代推注式硅胶人工晶状体植入术无须扩大切口 ,容易掌握 ,术后反应轻 ,并发症少 ,手术疗效满意。

Objective To investigate the clinical results of advanced generation silicone intraocular lens(IOL)implanted with injector.Methods Advanced generation silicone IOL was implanted with injector in group A(102 eyes)and the PMMA IOL,implanted in group B(108 eyes)as the control.The changes of visual acuity,corneal astigmatism,anterior chamber flare,corneal edema,location of IOL,posterior capsular opaqueness and other operation complications were observed.Results Postoperatively,the uncorrected visual acuities were 0.5 or better in 70.3%(group A)and 52.8%(group B)at 1st week,the visual acuities with spherical correction were 1.0 or better in 71.8%(group A)and 55.4%(group B)at 1st month,and in 70.3%(group A)and 53.6%(group B)at 3rd months respectively.There were significant differences between the group A and group B.The mean diopters of corneal astigmatism compared with that before the surgery had no significant difference in the group A,and had significant difference in the group B,at 1st month after the operation.The incidents of anterior chamber flares,corneal edema,posterior capsular opaqueness and subluxation of IOL of group A were significantly lower than that of group B.The rate of IOL damaged and reversed in the anterior chamber were 2.9% and 1%,respectively in group A.Conclusions The advanced generation silicone IOL implanted with injector through an un-enlarged incision,easy to master,with less operation complications and shorter recovery period.The postoperative therapeutic effects are satisfactory.
