摘要
目的 探讨眼压与非对称性糖尿病视网膜病变颈部及球后血管血流动力学之间的相互关系。方法 ① 12例患者 (内分泌科诊断为 2型糖尿病 ;无高血压、心脑血管病及血液病等全身病 ;结合直接检眼镜和眼底荧光血管造影诊断为非对称性糖尿病视网膜病变 ) ,每位患者较重眼归为一组 ,较轻眼归为一组 ,共两组。②采用日本产TOPCON自动眼压仪测量眼压 ;采用美国产HDL -5 0 0 0型彩色多普勒仪检测患者颈部及球后血管血流动力学变化。③统计学处理 (SPSS10 0版统计软件 ) :两组眼压采用配对t检验 ;眼压与同侧血管血流动力学参数做相关分析。结果 ①较轻组眼压为 16 5 8± 1 5 1mmHg ,较重眼组为 18 42± 1 62mmHg ,两者相比有统计学差异 (P <0 0 1) ;②颈内动脉(ICA) :较轻组ICA收缩期峰值血流速度 (PSV )为 47 47± 8 62cm/s ,与眼压无相关性 (r =-0 160 ,P =0 619) ;较重组PSV为 43 63± 9 16cm /s ,与眼压无相关性 (r =-0 5 2 9,P =0 0 77)。③眼动脉 (OA) :较轻组PSV为 2 7 2 7± 12 2 0cm/s ,与眼压呈负相关 (r =-0 60 0 ,P =0 0 3 9) ;较重组PSV为 2 0 0 2± 5 3 2cm/s ,与眼压呈负相关 (r =-0 72 1,P =0 0 0 8)。④视网膜中央动脉(CRA) :较轻组PSV为 8 41± 2 0 5cm /s ,与眼压呈负相关 (r
Objective To investigate the relationship between the intraocular pressure(IOP) and the hemodynamic changes in cervical and retrobulbar vessels in patients with asymmetric diabetic retinopathy.Methods The eyes with asymmetric diabetic retinopathy were divided into the severe eyes group and the better eyesgroup.TOPCON auto-ophthalmotonometer was used to quantitate the intraocular pressures.Color Doppler imaging(CDI) was used to quantitate the peak systolic velocity(PSV) of internal carotid artery(ICA),ophthalmic artery(OA),central retinal artery(CRA),the velocity(V) of central retinal vein(CRV).Results A significant increase was shown in the IOP of the severe eyes group(18.42±1.62mmHg)compared to the better eyes group(16.58±1.51mmHg)(P<0.01).There was a negative correlation between the IOP and PSV in OA in both the better eyes group(PSV:27.27±12.20cm/s,r=-0.600,P=0.039) and the severe eyes group(PSV:20.02±5.32cm/s,r=-0.721,P=0.008).There was a negative correlation between the IOP and PSV in CRA in both the better eyes group(PSV:8.41±2.05cm/s,r=-0.677,P=0.016)and the severe eyes group(PSV:6.44±1.78cm/s,r=-0.661,P=0.019).Conclusions A negative correlation was shown between the IOP and the PSV in OA and CRA.The asymmetric bilateral IOP may be one of te causes resulting in the asymmetric diabetic retinopathy.
