睾丸扭转的诊断与治疗(附15例报告)

Diagnosis and treatment of testicular torsion(Report of 15 cases)

目的 探讨睾丸扭转诊断及治疗方法选择。方法 结合临床症状和体征确诊 15例 16侧睾丸扭转 ,14例手术治疗 ,1例手法复位 ,结合文献进行分析。结果 14例行急诊睾丸探查术 ,其中 8例行睾丸切除术 ,1例行手法复位 ,计有 8例睾丸保留。睾丸切除率达 5 0 %。随访 3～ 12月 ,复位及对侧睾丸形态和功能正常。结论 对睾丸扭转的早期诊断及早期开放手术是提高睾丸保留率的有效方法。

Objective To study the ways of diagnosis and treatment of testicular torsion and evaluate the therapeutic efficacy. Methods\ 15 cases of unilateral testicular torsion diagnosed in accordance with their symptoms and signs from December 1991 to April 2003 among which 14 cases treated with operation and 1 case with manual detorsion were reviewed and analyzed. Results\ Exploratory operations were performed for14 cases of unilateral testicular torsion,among which 7 cases underwent orchidopexy when the testis regainsed its blood supply after detorsion, and the other 8 cases had their necrotic testis cut. Only one case had manual detorsion. Testis cut rate was 50%(8/16). Conclusion\ All cases with suspected torsion should be given surgical exploration in early stage. It can increase salvage rates.
