摘要 目的 :检测血管内皮生长因子 ( VEGF- C)在宫颈癌组织中的表达及其与宫颈癌各病理、临床参数的关系 ,为预测宫颈癌转移、评价预后及合理治疗提供科学依据。方法 :应用免疫组织化学法对 5 7例浸润性宫颈癌、1 0例正常宫颈组织、1 2例宫颈原位癌、8例宫颈上皮不典型增生患者分别检测 VEGF- C的表达情况。结果 :VEGF- C在浸润性宫颈癌、宫颈原位癌和上皮不典型增生、正常宫颈组织中的表达率分别为 5 0 .9%、1 5 %、1 0 % ;VEGF- C的阳性表达率在有淋巴结转移组中为 85 % ,无淋巴结转移组中为 32 .4% ,差别有显著性 ( P<0 .0 5 ) ;在有深肌层浸润组的宫颈癌中 ,VEGF- C阳性表达率为 68.7% ,浸润深度未达到深肌层者的表达为 40 .6% ,二者间存在显著性差异 ( P<0 .0 5 )。结论 :VEGF- C的表达与浸润性宫颈癌淋巴结转移、间质浸润明显相关 ,而与临床分期、病理类型、病理分级、瘤体大小、患者年龄等无关 ;VEGF- C表达异常可作为评估宫颈癌浸润深度、淋巴结转移状况。 Objective:To study the expression of VEGF C in cervix neoplasms and its relationship with the clinicopathological features so as to predict the metastasis of cervix neoplasms, to evaluate prognosis and to manage a proper treatment .Methods:Immunohistochemical techniques were used to detect the expression of VEGF C in paraffin samples of 57 cases with invasive cervix neoplasms, 10 cases with normal cervical tissue, 12 cases with carcinoma in situ and 8 cases with atypical hyperplasia of cervical epithelia respectively. Results:The positive expression ratios of VEGF C in normal cervical tissue, in CIN(carcinoma in situ and atypical hyperplasia)and in invasive cervix neoplasms were 10% , 15% and 50.9% respectively. There was a significant difference between the expression of VEGF C in tissues with lymph atic metastasis and in those without, with the expression ratios being 85% and 32.4% respectively ( P <0.05). The expression ratios of VEGF C in samples of cervix neoplasms with invasion of deep muscular layer and of superficial muscular layer were 68.7% and 42.5% respectively. It revealed statistical significance ( P <0.05). The lower of differentiation grade, the later the clinical stage of cervix neoplasms, then the higher the positive expression ratios of VEGF C will be found, but there was no statistical significance ( P >0 05). Conclusion:The expression of VEGF C in invasive cervix neoplasms is significantly correlated with lymph atic metastasis and interstital invasion, but not with clinical stage, pathological grade, histological type, tumor size and patient age. The overexpression of VEGF C can be used as a useful index in treatment and prognosis of cervix neoplasms.
