延长部份心肺转流治疗体外循环心脏手术后右心衰竭

PROLONGED PARTIAL HEART-LUNG BYPASS FOR RIGHT HEART FAILURE AFTER EXTRACORPORAL CIRCULAT-ON HEART SURGERY

心肌缺血性损伤是体外循环心脏术后右心衰竭的主要原因,一些严重患者往往很难脱离体外循环机。膜式肺单泵驱动延长部份心肺转流可以减轻右心前负荷,维持心肌氧供和氧耗平衡,是一种治疗术后右心衰竭的机械辅助循环方法。本文报道了2例术后右心衰竭患者,使用部份心肺转流治疗分别长达5天和7天,最终康复。

Myocardial ischemia is the main cause of right heart failure after extracorporal circulation heart surgery.Some patients can not even be weaned from heart-lung machine.Prolonged partial heart-lung bypass using membrane oxygenator driven by single pump to reduce the preload of right ventricle and maintain the balance between myocardial oxygen supply and consumption is a therapy to be used as one kind of assisted circulation for postoperative refractory right heart failure. In this paper, two cases of postoperative right heart failure are presented. They were treated by the bypass technique for 5 and 7 days respectively, and both of them recovered very well in time.
