摘要 目的 :探讨牵引法、斜扳法治疗腰椎间盘突出症的力学依据。方法 :取无退化病变之新鲜尸体标本 ,用水银压力传感器 ,拉力电传感器和 SG-1型静态电阻应变仪 ,模拟牵引、牵抖、斜扳疗法下 ,分别测试椎间隙压力和前后纵韧带张力。结果 :牵引下 L4-5 S1椎间隙负压最大 ,前纵韧带张力大 ,后纵韧带处松弛状态 ,棘突旁加压或牵抖后 ,椎间负压加大 ,后纵韧带张力增强。斜扳法椎间盘负压无改变 ,但后纵韧带张力加大 ,论证了圆筒枢纽学说之骨盆圆筒牵引力作用线理论 ,和牵引法、斜扳法、牵抖法的适应症和应用注意事项。 Objective:To investigate the mechanics foundations for treatment of prolapse of lumbar intervertebral disc with oblique-pulling method.Methods:To measure the pressure between the spaces of intervertebral discs and the tension of anterior and posterior longitudinal ligaments using mercury pressure sensor, pulling electric sensor and static resistance strain gauge through the method of analogue pulling, shaking and oblique-pulling with newly anatomized corpse without retrograde affection as specimen.Results: The negative pressure between spaces of L 4-5S 1 intervertebral disk is the highest, the tension of anterior longitudinal ligament is much higher, and posterior longitudinal ligaments are in relaxation. will be enhanced if you pressurize near the spinous process. There are no changes of negative pressure between intervertebral disk under oblique-pulling method, but the tension force of posterior longitudinal ligament was increased. From those results, the action line theory of pulling force in pelvises cylinder about cylinder pivot motion theory was demonstrated. And it also demonstrated that the so-called horizontal pulling is the flexure along the action line, i.e. stretching instead of the vertical pulling.
