摘要 目的 研究白细胞介素 18(IL - 18)联合大剂维生素C(VitC)治疗病毒性心肌炎 (VMC)的疗效。方法 用柯萨奇病毒B3亲心肌株 (CVB3m)感染Balb/C小鼠制备VMC小鼠模型 ,然后分别给予IL - 18、大剂量维生素C、IL - 18联合大剂量维生素C治疗 8d ,观察小鼠发病情况 ,并在不同时段检测心肌组织病毒滴定度、心肌病理积分 ,并进行比较。结果各治疗组较心肌炎对照组症状减轻 ,生存率提高 (χ2 =10 .4 6 ,P <0 .0 5 ) ,心肌病毒滴定度降低 ,病理改变减轻 ,且有显著性差异 (P <0 .0 1) ;联合治疗组与IL - 18组比较 ,前者心肌坏死及炎性细胞浸涧减少 (q =4 .4 0 ,P <0 .0 5 ) ,但生存率及病毒滴定度无显著性差异 (P >0 .0 5 )。结论大剂量VitC及IL - 18对VMC均有一定疗效 ,但联合应用更能有效地防御病毒入侵、保护心肌。 Objective:To study the effect of interleukin-18 (IL-18) combined with high dosage vitamin C on viral myocarditis.Methods:85 Balb/C mice infected with Coxsachievirus B3 (CVB3) were divided into four groups at random. 20 cases of them were given periodically murine IL-18(0.1 microg/day. mouse ) for 8 days(group IL-18); 20 case of them were given high dosage vitamin C (200 mg/kg.mouse) for 8 days(group vitamin C); 20 case of them were given IL-18 combined with vitamin C(group combined) and 25 case of them were only given 0.2 ml normal saline (control group). The viral titer of the hearts was detected on day 5 and 8, and the myocardial inflammatory change was observed on pathologic pictures on day 5, 8 and 15.Results:The 14-day survival rate of each treated group was higher than that of the control group (χ2>10.46, P<0.05). But the mice in group combined didn't survive longer than the ones in group IL-18 (χ2>0.05). Compared with group IL-18 and group vit C, the pathological integral and the viral titer of the hearts of group combined on day 5 and 8 were significantly lower(P<0.01). Mice of group combined had less myocardial necrosis and celluar infiltration than those of the other three groups and there was significant difference(P<0.01).Conclusions:Both IL-18 and high dosage of vitamin C can prevent virus from damaging myocardium, but the combined use of them is superior to either of them.
