Expression of PCNA and Ki-67 in Thyroid Neoplasms and Their Correlation

摘要 目的 研究 (PCNA)、Ki-67增殖抗原在甲状腺肿瘤中的表达及其关系 ,并探讨它们在鉴别甲状腺良、恶性肿瘤中的诊断价值。方法 应用免疫组化EnVisionMT法对 76例甲状腺肿瘤细胞的PCNA和Ki-67进行检测。结果  3 9例甲状腺腺瘤PCNA指数和Ki-67指数分别为 0 16± 0 0 9( 3 9/ 3 9) ,1 49± 1 15 ( 3 9/ 3 9) ;3 7例甲状腺腺癌PCNA指数和Ki -67指数分别为 0 43± 0 3 1( 3 7/ 3 7) ,2 13± 1 5 2 ( 3 7/ 3 7) ,PCNA指数和Ki-67指数在腺瘤组和腺癌组之间比较均有显著性差异 (P <0 0 5 )。PCNA指数和Ki -67指数呈正相关 (rs =0 2 61,P <0 0 5 )。结论 PCNA指数可作为鉴别非嗜酸性甲状腺良、恶性肿瘤的参考指标 ,可能对甲状腺腺癌的生物学行为和预后评估有一定参考意义 ;Ki-67指数可作为滤泡性腺瘤和滤泡性腺癌鉴别诊断的依据。甲状腺乳头状腺瘤可能具有恶性潜能。 Objective To detect the expression of PCNA and Ki-67 proliferation antigens in thyroid neoplasms and to study diagnostic value of expression of PCNA and Ki-67 in benign and malignant neoplasms of thyroid. Methods The expression of PCNA and Ki-67 in 76 thyroid neoplasms was examined by immunohistochemical method(EnVision MT ). Results Among the 39 thyroid adenomas(TA), the indices of PCNA and Ki-67 were 0 16±0 09(39/39) and 1 49±1 15(39/39) respectively; Among the 37 thyroid carcinomas(TC), the indices of PCNA and Ki-67 were 0 43±0 31(37/37) and 2 13±1 52(37/37) respectively. The differences of both the PCNA and Ki-67 indices between TA and TC were significant(P<0 05). Correlations were observed between PCNA and Ki-67 indices (rs=0 261,P<0 05). Conclusions PCNA index may be helpful to identify benign and malignant neoplasms of thyroid of non-oxphillical cell, and to predict biological behavior and prognosis of thyroid carcinomas. Ki-67 index may be a diagnostic evidence for identifying follicular adenoma and carcinoma. Papillary thyroid adenoma may have malignant potential. High proliferation is the characterisitic of thyroid malignant neoplasms.
