不同时间窗尿激酶溶栓治疗急性脑梗死对照研究的Meta分析

Meta-analysis of comparative study of thrombolytic therapy for acute cerebral infarction with urokinase between various time window

目的 :了解尿激酶不同时间窗溶栓治疗急性脑梗死 (ACI)疗效的差异。方法 :应用Meta分析方法对在不同时间窗内使用尿激酶溶栓治疗ACI的 9项对照研究进行同质性检验和合并效应量的检验。结果 :①同质性检验 :χ2 =6 .12 74 ,自由度为 8,P >0 .0 5。②合并效应量的检验 :OR合并 =3.0 2 0 6 ,OR合并95 %可信区间为 1.776 1～ 5 .1371。OR合并 的检验 :χ2 =16 .6 4 79,P <0 .0 0 1。结论 :尿激酶 6h内溶栓治疗ACI的疗效显著优于 6h后的疗效。

Objective:To understand the difference of thrombolytic therapy for acute cerebral infarction with urokinase between various time window. Methods:9 studies, which compared the differences in effects of thrombolytic therapy for acute cerebral infarction with urokinase between various time windows, were analyzed with meta-analysis by approaches of homogeneity test and combined test. Results:(1)Homogeneity test shows that the cited studies are homogeneous with χ 2 =6.1274,P>0.05;and (2)combined test shows that the combined OR= 3.0206 and its 95% confidence interval was between1.7761～5.1371(χ 2=16.6479,P<0.001). Conclusion:The therapeutic effects of thrombolytic therapy with urokinase within 6 hours after onset of the symptom for acute cerebral infarction is significantly superior to those after 6 hours after onset of the symptom.
