乳果糖对急性肝损伤后肠源性内毒素血症防治作用的实验研究

Preventive Effect of the Lactulose on Gut-Originated Endotoxemia due to Acute Liver Injury

目的 探讨乳果糖对实验性肝损伤后肠源性内毒素血症的防治作用。方法 用硫代乙酰胺 (TAA)诱导大鼠急性肝损伤制作肠源性内毒素血症实验模型。选用雄性Wistar大鼠 5 8只 ,随机分为肝损伤组、乳果糖组和正常对照组 ,分别灌喂生理盐水 (NS)、乳果糖和NS各 2ml,2次 /d× 5d ,前二组第 4天皮下注射TAA 6 0 0mg/kg·只 -1,正常对照组仅注射NS ,4 8h后测定血内毒素、肿瘤坏死因子 (TNF)和丙氨酸氨基转换酶 (ALT)水平 ,并检测肠内容物细菌和内毒素含量。结果 和正常对照组比较 ,肝损伤组血内毒素和ALT水平显著升高 (P <0 .0 1) ;肠道G-杆菌菌量和肠内容物内毒素水平明显升高 (P <0 .0 1)。乳果糖组肠道G-杆菌菌量、肠内容物内毒素含量、血内毒素和ALT水平均明显低于肝损伤组 (P <0 .0 5 )。结论 乳果糖对肝损伤后肠源性内毒素血症具有防治作用。

Objective To study the preventive effect of the lactulose on gut-originated endotoxemia in experimental acute liver injury.Methods The experimental model was produced by thioactamide(TAA).Fifty-eight rats were divided into 3 groups:TAA group(A),lactulose group(B)and normal control group(C).Animals in group A and B were respectively feed with normal saline or lactulose 2 ml,Bid×5 days.At the fourth day,these animals were given TAA 600 mg/kg sc to induce acute liver injury.After 48h of TAA injection,the endotoxin,tumor necrosis factor(TNF)and ALT in blood were measured.Also, the bacteria counting and endotoxin levels of intestinal content were measured.Results Compared with normal control,the levels of endotoxin,TNF and ALT in blood were significantly increased in group A(P<0.01),bacteria counting of intestinal contents was evidently increased(P<0.01);In addition, these was an increased level of endotoxin in the intestinal content(P<0.01).However, the levels of endotoxin,TNF and ALT in blood,the bacteria counting and endotoxin levels of intestinal contents in group B were markedly lower than that in group A(P<0.05).Conclusion Lactulose present preventive and therapeutic effects on gut-originated endotoxemia in experimental acute liver injury.
