内给氧对ANP大鼠胰腺病理和微循环形态的影响 被引量:5

Effects of Intra-venous Oxygen Therapy on Pancreatic Pathology and Microcirculatory Morphology in Rats with Acute Necrotizing Pancreatitis

摘要 【目的】了解急性坏死性胰腺炎 (ANP)大鼠早期胰腺微血管的超微结构变化以及内给氧处理后的改变 ,并探讨内给氧与ANP病理改变的关系。【方法】采用逆行穿刺胰胆管并注射 5 %牛磺胆酸钠诱发大鼠ANP作为动物模型 ,分别观察假手术对照组、ANP组和内给氧组大鼠处死前腹腔内组织大体结构、并进行胰腺病理学评分和扫描电镜观察。【结果】①ANP组有胰腺坏死、胃肠扩张明显伴众多皂化斑和大量深红色血性腹水等改变 ,内给氧组上述各项有不同程度的减轻 ;胰腺的病理学评分内给氧组比ANP组明显降低 (P <0 .0 5 ) ;②扫描电镜观察 :与ANP组比较 ,内给氧组大鼠胰腺毛细血管间吻合的减少相对减轻 ,存在泡状物和树脂聚集相对较少及盲端数量减少等现象。【结论】ANP早期即有胰腺微血管损伤及可证实的病理改变 ,内给氧处理后胰腺病理和微血管损伤有不同程度的改善 ,推测其能为ANP治疗提供新的途径。 Objectives]To understand the early ultrastrucural changes of pancreatic microvessels and the alteration after intra-venous oxygen therapy in rats with acute necrotizing pancreatitis (ANP) as well as explore the relation of this therapy to pathological alteration during ANP.ANP models were established in Sprague-Dawley rats by retrograde puncture into pancreaticobiliary duct and injection of 5% sodium taurocholate. Inspection of the gross structures of intra-abdominal tissues before decapitation of the rats , evaluation of pathological scores in pancreas and scanning electron microscope were carried out in control rat group of sham-operation, ANP rat group and rat group of intra-venous oxygen therapy, respectively.①Pancreatic necrosis, apparent bowel dilatation accompanied with numerous saponified patches and accumulation of massive dark-red bloody ascitic fluid were found in ANP rat group ; the above-mentioned changes relieved to some extent in rat group of intra-venous oxygen therapy. The pancreatic histopathologic scores in rat group of intra-venous oxygen therapy were significantly lower than those in ANP group (P<0.05); ②Compared with the findings of scanning electron microscope in ANP group, rats in group of intra-venous oxygen therapy displayed rather slighter decrease of pancreatic inter-capillary anastomosis, existence of lesser bubble-like substance and resin aggregation, and reduced number of caeca, etc.[Conclusion]Pancreatic microvessel damages and some confirmable pathological changes already appear in the early stage of ANP. Intra-oxygen injection can ameliorate in certain degree the pancreatic pathology and microvessel damages, so it is speculated that this therapy will provide a new approach for treatment of ANP.
