ROLE OF CASPASE-1 ACTIVATED CYTOKINES IN ACUTE LUNG INJURY IN EXPERIMENTAL SEVERE ACUTE PANCREATITIS

摘要 目的 探讨Caspase 1激活的细胞因子在实验性重症急性胰腺炎 (SAP)肺损伤中的作用机制。方法 采用 5 %牛磺胆酸钠逆行注入胰胆管诱发大鼠SAP模型。SD大鼠 32只 ,随机分为 4组 :正常对照组 (HC组 )和SAP 6h组、SAP 12h组、SAP 18h组。采用ELISA法测定血清IL 1β水平 ;半定量RT PCR检测肺内Caspase 1、IL 1β及IL 18mRNA的表达。结果 SAP造模后各组血清IL 1β水平与HC组相比均显著升高 (P <0 0 1) ;HC组肺内可见Caspase 1、IL 1β及IL 18mRNA表达 ;SAP各组肺内Caspase 1、IL 1β及IL 18mR NA的表达显著上调 (P<0 0 1) ,而且与急性肺损伤 (ALI)的严重程度有关。结论 Caspase 1的激活、IL 1β及IL 18的过度表达在SAP并发ALI和ARDS过程中具有重要的作用。 Objective To study the role of Caspase-1 activated cytokines in acute lung injury (ALI) in experimental severe acute pancreatitis (SAP). Methods A rat model of SAP was induced by retrograde infusion of 5% sodium taurocholate into the biliary pancreatic duct. Thirty-two SD rats were randomly divided into four groups: healthy control (HC), SAP 6h, SAP 12h and SAP 18h groups. Serum IL-1β level was measured by ELISA. Intrapulmonary expressions of Caspase-1, IL-1β and IL-18 mRNA were detected by semi-quantitative RT-PCR. Results The serum IL-1β levels were significantly increased after the induction of pancreatitis (P<0.01, vs HC). Although RT-PCR showed intrapulmonary expressions of Caspase-1, IL-1β and IL-18 mRNA in HC, they were increased significantly in taurocholate-induced SAP (P<0.01, vs HC), in proportion with the severity of acute lung injury (ALI). Conclusion Caspase-1 activation, and the overproduction of IL-1β and IL-18 in the lungs play an important role in the course of ALI and ARDS during experimental SAP.
