卵巢肿瘤的彩超诊断和血管内皮生长因子的表达 被引量:1

摘要 目的 探讨彩超检查时卵巢良、恶性肿瘤的形态结构和血流动力学特点 ,以及血管内皮生长因子(VEGF)在肿瘤组织中的表达 ,旨在了解彩超诊断卵巢肿瘤的理论基础。方法 对 15 2例临床疑诊为卵巢肿瘤的患者行经阴道彩超检查 ,观察肿瘤的形态结构、血流信号分布及多普勒血流特征 ;对手术病理证实的 12例恶性肿瘤及 12例良性肿瘤的组织切片应用免疫组化SABC法检测VEGF的表达。结果 ① 4 9例恶性肿瘤和 99例良性肿瘤经手术病理证实 ;②恶性肿瘤组以囊实混合回声多见 ,瘤内血流信号丰富 ,且以低阻力的动脉血流为主 ;良性肿瘤组以囊性回声或边界规则的实性回声多见 ,以周边分布的高阻力血流信号为主 ,两组间比较差异均有显著性(P <0 .0 1,P <0 .0 5 ) ;③恶性组VEGF阳性表达率明显高于良性组 (P <0 .0 5 )。结论 经阴道彩超检查对术前诊断卵巢的良、恶性肿瘤有重要意义 ;组织中VEGF的表达与卵巢恶性肿瘤的发生、发展密切相关 ,以VEGF为靶点的抗血管生成的治疗是可行的。 Objective To study the morphologic and hemodynamic characteristics of ovarian tumors with Color Doppler sonography and the expression of vascular endothelial growth factor (VEGF) in the tissues.Methods One hundred and fifty-two patients with clinically suspected ovarian masses were evaluated by transvaginal sonography and Color Doppler sonography(TVS-CDS).A morphologic assessment was initially performed,followed by color flow Doppler analysis.SABC immunohistochemical method was used to investigate the expression of VEGF in 12 samples of malignant and 12 samples of benign tumors.Results ①Forty-nine malignant and ninty-nine benign tumors were pathologically confirmed.②Malignant tumors were more likely to have either complex or solid echogenic pattern and increased color flow was found in the echogenic portion and more presence of arterioles with low impedance.Benign tumors were more likely to have cytic echogenic pattern with regular borders.Less color flow was found in the tumors and the dominance is color flow around the tumors.There were significant differences between the two groups in morphologic and color flow image(P<0.01,P<0.05).③The expression of VEGF in malignant tissues was higher than that in benign tissues(P<0.05).Conclusion TVS-CDS is of great clinical and practical value in distingwishing benign and malignant ovarian tumors,and VEGF in the tumor tissues is correlated with the occurrence and development of ovarian cancer.It is feasible to treat ovarian cancer with antiangiogeneses therapy targeting VEGF.
