手术治疗胆囊残部综合征12例

Surgical Treatment of Residual Gallbladder Syndromes:a Study of 12 Cases

目的 :提高对胆囊残部综合征的认识 ,探讨胆囊残留病变产生的原因及防治对策。方法 :回顾分析 1994年1月～ 2 0 0 2年 7月收治的 12例胆囊残部综合征病人的临床资料。结果 :12例病人均于 3a～ 10a前行过胆囊切除手术 ,术后首次出现症状的时间为 1月～ 8a。 11例经B超诊断为胆囊残部结石 ,1例诊断为胆总管结石。再手术时均证实有小胆囊存在 ,其大小为 4cm× 3cm× 2cm～ 8cm× 4cm× 3cm ,内含结石 1～ 5枚。手术后均痊愈出院。结论 :胆囊切除手术的潜在危险依然存在 ,胆囊残留病变也并不少见。在胆囊切除手术中 ,不仅要考虑到胆管损伤的防治 ,也应重视胆囊残留病变的问题 ,努力做好每一例胆囊切除手术。

Objective:To improve the understanding of residual gallbladder syndromes and discuss reasons of residual gallbladder lesions and methods of prevention and treatment.Methods:12 cases of residual gallbaadder syndromes treated from January 1994 to December 2002 were studied with retrospective mehtods.Results:In 12 cases undergoing cholecystectomy before 3 to 10 years,manifestations in the upper right abdomen occurred from one month to eight years after the operation.11 cases were diagnosed as residual gallbladder stones by Bus and 1 cases as choledocholithiasis.All were proved that the gallbladder still existed on operation.The sizes of the residual gallbladder varied from 4cm×3cm×2cm to 8cm×4cm×3cm,and there are 1 to 5 gallstones in it.All were cured after the resection of residual gallbladders.Conclusion:The potential dangers exist still for cholecystectomy and the residual gallbladder lesions .During the surgical resection of the gallbladder,not only the injury of the bile duct but the residual gallbladder should be avoided carefully as well.We should try to do every cholecystectomy successfully.
