摘要
目的 对宫颈鳞状上皮不成熟化生中人乳头瘤病毒 (hum an papillomavirus,HPV )感染和增殖细胞核抗原 Ki6 7表达进行研究。 方法 分别从 15例宫颈鳞状上皮不成熟化生、 11例正常宫颈、 114例宫颈上皮内瘤变 (cervical intraepithelial neoplasia,CIN) ,9例 CIN ,10例 CIN 的石蜡标本提取基因组 DNA ,选取 HPV通用引物及型 ,特异引物进行多聚合酶联反应 (polymerasechain reaction,PCR)扩增 ,Ki6 7免疫组化染色。 结果 HPV总阳性检出率为 6 7.8% (40 /5 9) ,其中不成熟化生组 HPV感染检出率为 4 6 .7% (7/15 ) ,CIN 组 ,CIN 组 ,CIN 组均为 10 0 .0 %(14 /14 ,9/9,10 /10 ) ,正常组未检出 HPV感染病例 ,Ki6 7指数随 CIN级别的升高而明显增加。从正常宫颈组至 CIN 、 CIN 、 CIN 呈现逐渐增高趋势 ,差异有显著性 (F=5 6 .6 85 ,P<0 .0 5 )。除不成熟化生组与 CIN 组相比 ,差异无显著性 (P>0 .0 5 )外 ,其余各组两两比较 ,差异均有显著性(P<0 .0 5 )。 结论 HPV型别及 Ki6 7指数与 CIN级别密切相关 ,高危型
Objective To study human papillomavirus(HPV)infection and expression of Ki67 antigen in cervical squamous immature metaplasia. Methods HPV typing by polymerase chain reaction and Ki67 antigen immunostaining were conducted in 15 cases with squamous immature metaplasia,11 with normal cervix,14 CIN Ⅰ,9 with CIN Ⅱ and 10 in CIN Ⅲ. Results HPV infection was not detected in cases with normal cervix.7 out of 15 cases with immature metaplasia were positive for HPV detecting,3 of them were positive for HPV 16/18 typing.HPV 16/18 typing were found in 5 of 14 cases with CIN Ⅰ.44 4 % and 50 % positive of HPV 16/18 were observed in cases of CIN Ⅱ and CIN Ⅲ,respectively.Ki67 indices significantly elevated as CIN grade increasing.The range of Ki67 indices in immature metaplasia covered from normal cervix to CIN Ⅱ. Conclusion Our study suggested that HPV typing and Ki67 expression were closely associated with CIN grade.High risk typing of HPV may playan important role in sqamous immature metaplasia.
