保留下颌骨骨折线上牙齿的相关临床研究 被引量:8

摘要 目的 :对下颌骨骨折线上牙齿的保存率相关因素进行分析研究。方法 :收集我院 10年间下颌骨骨折 98例 ,并对 16 6个骨折线上的牙齿进行临床分类 ;将针对与牙齿保存率相关因素进行分析。结果 :将下颌骨骨折线上的牙齿分为四型。其中Ⅱ型最多 ,16 6个骨折线上的牙齿中 14 0个被保留下来 (84 .3% ) ;保存率最高为Ⅰ型。拔除牙齿多与牙损伤严重、骨折片移位较大并伴有骨缺损、牙周病引起的骨进行性吸收、治疗不及时或方法不当等因素有关。结论 :积极有效地治疗方法可以提高下颌骨骨折线上牙齿的保存率 ,而保留骨折线上的牙齿对下颌骨骨折的复位固定 ,恢复良好的咬合关系及骨折愈合等都是非常重要的 ; Objectives:To analyze the related factors of the teeth on the lines of mandibular fractures were preserved.Methods:One hundred sixty-six teeth on the fracture lines of mandibular fractures (98 patients) treated at the hospital, from1991 to 2001, were clinic statistically analyzed and classified. To study the rate of teeth reservation related factors such as the cause of frcture, the site, state of the teeth on the fracture lines and treatment.Results:Teeth on the mandibular fracture lines could be classified into four types (Ⅰ~Ⅳ) according to our original classification; typeⅠwas 41 cases, type Ⅱ was 82 cases, type Ⅲ was 30 cases, and type Ⅳ was 13 cases. One hundred forty (84.3%) out of all teeth were preserved without teeth extractions. The teeth were extracted closely related to the large displacement of fractures with mandibular defect, periodontal disease and the complexity of the teeth defect and the delayed treatment or not very suitable therapy.Conclusions:The classification of teeth on the lines of mandibular fractures was instructive significance to the treatment and prognosis of teeth on the fracture lines. In addition active treatment could be improve the rate in the reservation of teeth on the lines of mandibular fractures.
