缩窄性心包炎心包切除范围的探讨

Effect resectional pericardium on constritive pericarditis operation

目的 :探讨缩窄性心包炎心包切除范围对手术效果的影响。方法 :选择 1980～ 1999年缩窄性心包炎病人 18例。依据手术方法不同分成两组 ;观察组 8例行左心包切除术 ,对照组 10例做全心包切除术。结果 :术后观察左心组的平均动脉压(MAP)为 (114± 11.6 7) m m Hg,心率 (HR) (99.5 0± 8.2 1)次 /分 ,脉压差 (PPD) (2 5 .6 3± 2 .97) m m Hg,均大于对照组 MAP(90 .80± 10 .6 6 ) m m Hg,HR(88.80± 8.6 3) m m Hg和 PPD(16 .70± 3.0 6 ) mm Hg。统计学处理 ,P <0 .0 1。结论

Objective: To evaluate the effect of resectional pericardium size on constrictive pericarditis operation. Methods: From 1980 to 2002, 18 patients with constrictive pericarditis underwent surgical treatment. Based on operative approaches the patients were divided into two groups,observation group (8 cases) and control group (10 cases). Only the left pericardiectomy was performed in the 8 cases in the observation group,whereas the whole petricardiectomy was perfomed in the locases in the control group. Results: After the operation,the mean artery pressure (MAP) (114±11.67)mmHg, heart rate (HR) (99.50±8.21) time every min and pulse pressure difference (PPD) (25.63±2.97)mmHg were all much higher in the observation group than those in the control one MAP(90.80±10.66)mmHg, HR (88.80±8.63)time every min and PPD (16.70±3.06)mmHg (P<0.01). Conclusion: The left pericardiectomy in pericarditis is much better than the whole pericardiectomy in the heart function recovery.
