圆筒形骨凿在口腔颌面外科的临床应用

Application of cylindrical osteotomes for extraction of bone transplants in oral and maxillofacial surgery

目的 探讨骨移植用圆筒形骨凿在口腔颌面外科的临床应用。方法 随机将97名需行自体髂骨骨松质移植的患者分为2组,由同一术者分别采用圆筒形骨凿通过小切口和常规切口髂骨开窗松质骨刮取术切取自体髂骨骨松质,分别记录患者的伤口长度、伤口愈合情况、卧床时间、感觉障碍、疼痛、步态障碍等,并进行统计学处理。结果 2组患者在伤口长度、卧床时间及感觉障碍、疼痛、步态障碍的差异有非常显著性(P<0.01)。结论 运用骨移植用圆筒形骨凿通过小切口切取自体髂骨骨松质,创伤小,不失为一种良好的方法。

Objective To analyze the application of cylindrical osteotomes for extraction of bone transplants in oral and maxillofacial surgery. Methods 97 patients who needed to receive self-iliac cancellous bone graft were randomly divided into 2 groups; Cancellous bone was extracted by cylindrical osteotomes through mini-incision in one group and by routine method in the other. The same doctor did all these operations. The length and healing condition of wound, the time of lying in bed, sensory disability, pain, gait disorder and so on were recorded and treated by statistics. Results Between the two groups, the differences among the length of wound, the time of lying in bed, sensory disability, pain, and gait disorder were very significant. Conclusion It was a microtrauma to extract cancellous bone from iliac spongy substance of bone through mini-incision by cylindrical osteotomes. It was a satisfactory method for bone graft.
