应用双颌手术矫治严重的颌面部畸形

The usage of bimaxillary surgery in the correction of serious oral-facial deformity

目的:探讨双颌手术在矫治颌面部畸形中的经验和体会。方法:应用上颌骨Lefort-Ⅰ型截骨术,下颌骨矢状劈开截骨术和水平截骨颏成形术,Ⅰ期完成多个颌面部畸形矫治,达到改善容貌和恢复正常咬合关系的目的。结果:31例颌面部畸形病例,均按上述方法完成矫治,治疗结果通过开口度、开口型、面形、咬合关系、咀嚼力等五个方面进行评估,治疗结果令人满意,无严重并发症。结论:双颌手术是一种有效的治疗颌面部畸形的方法,应成为矫治颌面部畸形的常规方法。

Objective To assess the effects of bimaxillary surgery in the correction of oral-facial deformity function, occlusion, aesthetic and complications treatment. Methods 31 cases (averaged 19 years old) were treated with bimaxillary surgery (Lefort- I osteotomy, sagittal split ramus osteotomy SSRO, and broad-pedicle genioplasty) .mouth-open degree, mouth-open pattern type, facial aesthetic type, occlusion and masticatory force were assessed pre-Surgery and post-surgery. Complications were recorded and manipulated. Results Maximal mouth-open degree averaged 28.9mm six months after surgery and no mouth-open deviation was observed. Normal occlusion and masticatory force were achieved. Facial aesthetic were improved greatly, without severe complications occurred. Conclusion Bimaxillary surgery was proved to be effective in the correction of oral -facial deformity and recommended to be routine operation.
