摘要 目的 探讨KAI1基因与大肠癌转移及预后的关系。方法 采用免疫组织化学检测 91例大肠癌原发灶及 2 5例淋巴结转移灶的KAI1表达。结果 KAI1在大肠癌原发灶中表达阳性 5 4例 ( 5 9.3 %) ,弱阳性 2 2例 ( 2 4.2 %) ,阴性 15例( 16.5 %)。KAI1在伴淋巴结转移、远隔器官转移的癌组织中表达显著降低 (P <0 .0 1) ;淋巴结转移灶的KAI1表达显著低于原发灶 (P <0 .0 5 ) ;5年生存者的KAI1阳性表达率显著高于死亡者 (P <0 .0 5 ) ;KAI1表达阳性、弱阳性以及阴性患者的 5年生存率 ( 87.0 4%;63 .3 4%;60 %)依次递减 ,有显著性差异 (P <0 .0 5 )。结论 KAI1的表达下调参与了大肠癌的淋巴结转移和远处转移 。 Objective To explore the relationship of KAI1 gene with metastasis and prognosis of colorectal cancer(CRC). Methods The expression of KAI1 in 91 cases of primary CRC and 25 cases with lymph node metastasis of CRC was detected by immunohistochemistry. Results Positive expression, weak expression, and negative expression of KAI1 were found in 54 (59 3%), 22 (24 2%), and 15 case (16 5%) of primary CRC, respectively. KAI1 expression decreased significantly in cases of CRC with lymph node metastasis and distant metastasis ( P <0 01). KAI1 expression in cases of CRC with lymph node metastasis was lower than that in the primary locus( P <0 05). The positive rate of KAI1 expression of 5 year survival patients was higher than that of the dead( P <0 05). The 5 year survival rate of patients with positive, weak, and negative expression of KAI1 decreased in turn ( P <0 05). Conclusion The down regulation of KAI1 expression may participate in the lymph node metastasis and distant metastasis of CRC, and may probably be the valuable marker to predict the prognosis of CRC.
