The Trapdoor Procedure Using Antogenous Cancelloous Bone Grafts for Treatment of the Femoral Head Necrosis

目的 报道成人非创伤性股骨头坏死的手术治疗。方法 非创伤性股骨头坏死 10例 ,平均年龄 39岁 ,Ficat分期Ⅱ～Ⅲ期。其中Ⅱa 3例 ,Ⅱb5例 ,Ⅲ期 2例。采用经股骨头负重区开瓣死骨清除 ,取髂骨植骨术 ,结果 平均随访 32个月。根据Harris临床评分 ,Ⅱb 以前 8例效果满意 ,优良率达 85 5 %。结论 该手术对于Ⅱb

Objective To present outcome of the necrosis of the femoral head treated by the trapdoor procedure using antogenous cancellous bone grafts.Methods We have reviewed the results of 10 cases of nontraumtic femoral head necrosis,with stage Ⅱ～Ⅲ,(Ⅱa 3,Ⅱ b 5,Ⅲ 2)according to Ficats classification.The necrotic bone was removed using osteotomes,curettage and the trapdoor procedure using antogenous cancellous bone grafts were performed.Results Ten cases were followed up at a mean of 32 months after operation.The results were excellent on 8 cases with Ficat stage Ⅱ b.The excellend and good rate was 85 5 percent.Conclusion We consider that the indication for trapdoor procedure are Ficat stage Ⅰ～Ⅱ b disease with a fair cartilage sphere.
