Analysis of Operative Complication of Displaced Acetabular Fracture

目的 探讨髋臼移位骨折手术并发症发生原因及防治措施。方法 对我院1988年4月～2002年1月手术治疗的42例移位髋臼骨折发生的20例并发症进行分析。结果 移位髋臼骨折手术并发症发生率为47.6％,其中,术中并发症:脂肪栓塞1例,坐骨神经损伤1例。术后并发症:创伤性关节炎9例,异位骨化7例,股骨头无菌坏死1例,内固定失效1例。结论 正确选择手术入路和内固定材料,提高手术技巧和解剖复位率,尽量减小剥离,合理的早期功能锻炼是防治移位髋臼骨折手术并发症的关键。

Objective To discuss the cause, preventive and curative measures of operative complications from displaced acetabular fractures. Methods It was analyzed, from April 1988 to January 2002, 42 cases of displaced acetabular fractures were treated by internal fixation. Rejults Of 42 cases, there were complications in 20 cases. The rate of operative complications from acetabular displacement fractures was 47.6% , 1 case fat embolism syndrome and 1 case sciatic nerve injury occurred in operation, but postoperative complications included of posttraumatic osteoarthritis in 9 cases, ectopic ossification in 7 cases, a vascular necrosis of the femoral head in 1 case and failure of internal fixation in 1 case. Conclusion It is the key of prevention and cure of operative complications from displaced ac-etabulum fractures, that is exact choice of surgery approach and material of internal fixation, to improve the skill of operation, anatomic reduction, less peeling off and logical early exercise.
