摘要
目的 :探讨胆囊癌患者红细胞补体I型分子 (CR1)及其基因多态性变化。方法 :采用PCR技术和HindIII酶切技术测定红细胞CR1分子基因型 ,采用酶联免疫法定量测定红细胞CR1分子的数量 ,采用红细胞天然免疫粘附试验测定红细胞CR1分子粘附活性。结果 :(1)胆囊癌患者手术前红细胞CR1分子数量A4 0 5值及CR1分子粘附活性均显著低于正常对照组、胆结石和胆囊炎组 (P均 <0 .0 1) ,胆结石和胆囊炎患者红细胞CR1分子数量A4 0 5值及CR1分子粘附活性亦低于正常对照组 (P <0 .0 5 ) ;CR1分子数量A4 0 5下降程度与CR1粘附活性下降程度相一致 ;(2 )与术前相比 ,手术后 3d ,胆囊癌患者红细胞CR1分子数量A4 0 5值及CR1分子粘附活性急剧下降 (P <0 .0 1) ;术后 1周较术后 3d ,红细胞CR1分子数量A4 0 5值及CR1分子粘附活性有所上升 ,但仍低于术前 (P <0 .0 1) ;术后 2周较术前 ,红细胞CR1分子数量A4 0 5值及CR1分子粘附活性已无明显差别 (P >0 .0 5 ) ;术后 3周较术前 ,红细胞CR1分子数量A4 0 5值及CR1分子粘附活性明显升高 (P <0 .0 1) ;(3)凡有局部浸润、肿瘤周围淋巴结转移、其它脏器转移的胆囊癌患者 ,其红细胞CR1分子数量A4 0 5值及CR1分子粘附活性较无这些改变的患者有不同程度的降低 (P均 <0 .0 1) ;(4) 31例胆囊癌患者?
Objective: To study the changes of quantitative expression, adhering activity and genomic density polymorphism of complement types in erythrocytes(CR1) of patients with gallbladder carcinoma. Methods: Polymerase chain reaction(PCR),Hind III restriction enzyme digestion,the quantitative assay of ECR1 and the adhering activity assay of CR1 in erythrocytes were used. Results:(1)The number and the adhering activity of ECR1 in patients with gallbladder carcinoma were significantly lower than those in chronic cholecystitis and gallstone and healthy individuals controls(P<0.01), the number and the adhering activity of ECR1 in patients with chronic cholecystitis and gallstone were significantly lower than those in healthy individuals controls(P<0.05); there was a good relationship between quantitative expression and the adhering activity of ECR1. (2)Compared with the preoperative day, the number and the adhering activity of ECR1 in patients with gallbladder carcinoma decreased greatly on the third postoperative day(P<0.01),the number and the adhering activity of ECR1 in patients with gallbladder carcinoma also decreased greatly in the first postoperative week (P<0.01), but they were increased slightly than those in the preoperative day(P>0.05). Compared with the preoperative day and the first postoperative week, the number and the adhering activity of ECR1 recovered in the second postoperative week(P<0.01). Compared with the preoperative day, the number and the adhering activity of ECR1 increased greatly in the third postoperative week(P<0.01). (3) The number and the adhering activity of ECR1 of gallbladder carcinoma patients with infiltrating, adjacent lymphogenous and distant organ metas-tasis were significantly lower than that of gallbladder carcinoma patients without them(P<0.01). (4) No difference was observed between the patients with gallbladder carcinoma and healthy individuals in the spot mutation rate of CR1 density gene(x 2=0.521, P>0.05).The number and the adhering activity of CR1 high expression genomic type(HH) gallbladder carcinoma were significantly lower than those of healthy individuals (P<0.01). Conclusion:Defective expression of CR1 in gallbladder carcinoma is mostly acquired through central peripheral mechanisms. The changes of CR1 quantitative expression and adhering activity are consanguineously related to the development and metastasis in gallbladder carcinoma.
