紫癜性肾炎患者血和肾组织P选择素的表达及意义

Research on the expression of P-selectin in renal tissue of patients with Henoch-Schnlein purpura nephritis

目的 探讨粘附分子P选择素 (CD62P)在紫癜性肾炎 (HSPN)患者中的致病作用。方法 以肾穿刺活检法取HSPN患者肾组织 ,采用直接免疫荧光标记单克隆抗体 ,常规和特殊病理染色 (苏木素 伊红染色、过碘酸雪夫染色、Masson三色染色、六胺银染色 ) ,外周血采用流式细胞术的方法进行了CD62P表型的研究。结果 ① 3 2例HSPN患者肾组织中显示CD62P表达 2 7例( 84 4% ) ,表达部位常见于肾小球系膜区、血管袢 ,其次为间质小血管、肾小管。病理类型的表达以Ⅲ、Ⅳ型显著 ,Ⅱ型次之 ,Ⅰ型最弱。② 3 2例HSPN患者外周血CD62P表达量较对照组显著增高 (P<0 0 1) ;CD62P在肾组织表达与外周血表达水平密切相关 ,且与肾脏病理类型的病变部位相一致。结论 CD62P在HSPN患者肾组织和外周血中表达增强 。

Objective To investigate the role of adhesion molecules P-selectin (CD62P) as a pathogenic factor in Henoch-Schonlein purpura nephritis (HSPN) patients.Methods The expression of adhesion molecules CD62P marked by fluorescent monoclonal antibody in renal biopsy tissue and peripheral blood was studied by flow cytometry in patients with HSPN.The renal tissue was collected and analyzed by light microscopy (HE,PAS,Masson,PASM).Results ① CD62P expressed in renal tissue of 27 cases among all 32 patients with HSPN (84.4%).It expressed mainly in glomerular mesangial area and capillary vessels,then,interstitial small blood vessels,renal tubule.CD62P most significantly expressed in type Ⅲ and Ⅳ,then the type Ⅱ and the last type Ⅰ.② Thirty-two patients of HSPN were highly expressed on CD62P in peripheral blood than those of control group (P<0.01).High expression in kidney tissues was associated with peripheral blood expression,and was coincident with involved areas the kidney.Conclusion CD62P′s high expression in renal tissue and peripheral blood suggested that CD62P might play an important role in the pathogenesis if HSPN.
