摘要 目的 研究宫颈鳞癌患者2 52 锎中子照射前后肿瘤细胞凋亡的变化 ,初步探讨2 52 锎中子治疗宫颈癌的机制及临床意义。方法 采用TUNEL法检测 2 0例宫颈鳞癌患者中子放疗前后的肿瘤细胞。结果 发现凋亡指数 (AI)和肿瘤细胞凋亡阳性的患者分别由放疗前的 ( 4 .2 2 %± 4.14 ) %和 3 5 % ( 7 2 0 )上升至放疗后的 ( 3 4.64± 2 4.3 6) %和 85 % ( 17 2 0 ) (P <0 .0 1) ,无论放疗前后 ,平均AI在各分期和各级间未见明显差别 (P >0 .0 5 ) ,经随访 ,全部患者中仅 1例Ⅲb期患者出现复发 ,再次给予放疗后控制良好。并发症主要为放射性直肠炎 ,发生率 10 %。结论 2 52 锎中子照射后可引起肿瘤细胞较强的凋亡反应 ,诱导细胞凋亡在2 52 Objective To investigate the changes of apoptosis of the cervical carcinoma cells before and after 252 Cf neutron brachytherapy and to explore the mechanism of 252 Cf neutron in brachytherapy and its clinical significance. Methods The apoptosis positive rate and apoptotic indexes (AI) in 20 cases of cervical carcinoma were detected before and after 252 Cf neutron brachytherapy by TdT mediated dUTP nick end labeling (TUNEL) assay. Results The mean AI and apoptosis positive rate increased from (4.22±4.14)% and 35% to (34 64±24 36)% and 85% respectively after radiotherapy ( P <0 01). No significant difference in mean AI was found in each stage or grade before and after radiotherapy ( P >0.05). Conclusion Enhanced apoptotic response of tumor cells can be resulted from 252 Cf neutron radiation. Induction of tumor cell apoptosis may be the important mechanism of 252 Cf neutron brachytherapy.
