微创刨吸术治疗下肢静脉曲张初步研究

Preliminary study of minimally invasive operation in treatment of varicose in lower limb

目的 探讨微创刨吸术治疗下肢静脉曲张的初步疗效。方法 对 2 0 0 3年 6～ 9月 4 0例 (5 3条肢体 )下肢静脉曲张病人进行手术治疗。 5 3条患肢均存在浅静脉曲张 ,16条患肢同时伴交通支静脉功能不全 ,10条患肢同时伴慢性静脉性溃疡 ,5条患肢同时伴重度深静脉返流。根据病情需要分别或同时行内镜深筋膜下交通支静脉结扎、大隐静脉高位结扎和剥脱飞股浅静脉瓣膜外修复成形术和下肢曲张浅静脉微创刨吸术。结果 术后 5 3条患肢肢体症状和浅静脉曲张消失 ,10条患慢性静脉性溃疡肢体短期内愈合。术后短期随访 ,无新生的溃疡 ,无静脉曲张复发 ,小腿外观无明显手术疤痕。结论 微创刨吸术治疗下肢静脉曲张安全有效 ,损伤少 ,并发症少 ,达到了微创和显效的目的 ,可能是治疗下肢慢性静脉功能不全的重要组成部分。

Objective: To evaluate the preliminary effect of minimally invasive operation treatment of varicose in the lower limbs. Methods: Between June.2003 and Sep.2002, 40 patients(53 limbs) with varicose vein underwent venous surgery. All of them had superficial varicose vein. Sixteen limbs had incompetence of traffic branch vein, ten limbs had chronic venous ulcers, and five limbs had severe deep venous reflux. Several surgical procedures were performed separately or simultaneity, depending on the individual, including subfascial endoscopic perforator ligation, flush saphenofemoral ligation, greater saphcnous vein stripping, external suture exfoliate vein valve repair, and minimally invasive operation treatment of varicose in the lower limbs.Results:The surgery obviously improved venous disability, varicose vein disappeared in all patients; ulcers healed shortly in ten limbs. There have been no ulcer recurrences at a new site and recurrent varicoscs during follow-up. There were no obvious scars on the legs. Conclusions:Minimally invasive operation is safe, effective, and minor injmy ,as well as lower complications in the wound. It probably plays an important role in the treatment of varicose in the lower limbs.
