首诊于眼科的颈动脉海绵窦瘘的临床分析

Clinical analysis of carotid cavernous fistula first examined in Ophthalmology

目的分析颈动脉海绵窦瘘的临床特征,提高对该病的认识。方法回顾分析3例颈动脉海绵窦瘘的误诊原因、临床特点、诊断、鉴别诊断和治疗。结果3例中,男2例,女1例,年龄24～46岁。2例有外伤史。分别被误诊为眼蜂窝织炎、青光眼、眼炎性假瘤。后经DSA或MRI确诊为颈动脉海绵窦瘘。2例患者经可脱球囊栓塞治疗均获得满意效果,1例未治。结论详细追问病史和体格检查可早期诊断该病,及时治疗预后较好。

ObjectiveTo analyze the clinical characteristics of carotid cavernous fistula and improve the diagnosislevel.Methods3 cases of carotid cavernous fistula were studied retrospectively, including the misdiagnosis causes, clinicalcharacteristics, diagnosis, differential diagnosis and treatment.Results2 patients were male and 1 patient was female. Theage ranged from 24 to 64. There were injury history in 2 patients. 3 cases were misdiagnosed to be blepharitis phlegmonosa,glaucoma or eye inflammatory pseudotumor. Finally they were confirmed by DSA or MRI. 2 cases which received trans-femoral detachable balloons surgery had achived satisfied effect.ConclusionsDetailed inquiry for medical history and bodyexamination are very important in diagnosis and treatment.
