肾下型腹主动脉瘤腔内隔绝术与传统手术对肾功能影响的比较研究

Comparative study on the effects of conventional surgery and endovascular graft exclusion on renal function of the patients with infrarenal abdonominal aortic aneurysms

目的 :对比肾下型腹主动脉瘤腔内隔绝术 (EVGE)与传统手术 (CS)对肾功能的影响。方法 :回顾1997年 3月至 2 0 0 2年 3月间实施的 15 7例肾下型腹主动脉瘤手术 ,其中EVGE组 115例 ,CS组 42例 ,分析比较二组手术前后血肌酐、血尿素氮的变化情况。结果 :CS组术后血肌酐、血尿素氮较术前明显升高 ,并发生急性肾衰 1例 ,而EVGE组手术前后两指标无显著差异。结论 :EVGE对肾下型腹主动脉瘤患者肾功能的影响明显小于CS。

Objective:To assess and compare the effects on renal function after endovascular graft exclusion(EVGE) and conventional surgery(CS) for infrarenal abdominal aortic aneurysms(IAAA).Methods:The records of 157 consecutive patients with IAAA from 1997 to 2002 were retrospectively reviewed.There was a group of 115 patients undergoing EVGE and a group of 42 patients undergoing CS.The postoperative changes of plasma Cr and BUN with EVGE and CS were analyzed respectively and compared.Results:While the plasma Cr and BUN significantly increased in the group of CS postoperatively,moreover,there was a case with acute renal failure in CS group,no significant difference between per- and post- endovascular repair was discovered in the group of EVGE.Conclusion:The effect of EVGE on renal function of the patients with IAAA is much less than CS.
