高血压肥胖患者血清和脂肪组织脂蛋白脂酶测定及临床意义 被引量:2

摘要 目的 探讨高血压肥胖患者血清、脂肪组织中脂蛋白脂酶含量与血脂紊乱、胰岛素抵抗、肥胖、高血压等代谢紊乱的关系。方法 选取住院的腹部手术患者 4 8例为研究对象 ,高血压肥胖患者及正常对照组各 2 4例 ,测定血压、体重指数、腰臀比、血脂、血糖、胰岛素以及血清和大网膜脂肪组织脂蛋白脂酶的含量。结果 高血压肥胖组患者的甘油三酯 (TG)、胰岛素 (INS)、空腹血糖 (FBS)均显著高于对照组 (P <0 .0 1) ,血清脂蛋白脂酶 (LPL)含量、高密度脂蛋白 (HDL)显著低于对照组 (P <0 .0 1) ,血清总胆固醇 (TC) (P >0 .0 5 )、低密度脂蛋白胆固醇 (LDL C) (P >0 .0 5 )无明显差别。高血压肥胖组大网膜脂肪组织LPL的含量与对照组无显著差别 (P >0 .0 5 )。血清LPL含量与体重指数 (BMI) (r =- 0 .6 4 0 ,P <0 .0 1)、腰臀比 (WHR) (r =- 0 .5 89,P <0 .0 1)、TG(r =- 0 .784 ,P <0 .0 1)负相关 ;与胰岛素敏感指数 (ISI) (r =0 .5 33,P <0 .0 1)、HDL(r =0 .4 6 3,P <0 .0 1)正相关 ,与TC(r =- 0 .2 2 2 ,P>0 .0 5 )、LDL(r =- 0 .2 13,P >0 .0 5 )无相关性 ;脂肪组织LPL含量与上述指标均无相关性 (P >0 .0 5 )。结论 肝素前LPL的含量与高血压肥胖患者的发病及代谢紊乱显著相关 ; Objective To investigate the relationship between serum lipoprotein lipase(LPL) content, great omentum adipose tissue lipoprotein lipase content and dyslipidemia,insulin resistance, obesity and hypertension in obesity-related hypertension patients. Methods The subjects who were inpatients and performed abdominal operation were selected in present study, 24 obesity-related hypertension and 24 age,sex-matched normal body mass individuals were studied. Blood pressure,body mass index(BMI),waist-hip ration(WHR),serum lipid,serum glucose,insulin(INS),LPL in serum and great omentum adipose tissue was measured in every subject. Results The values of triglyceride(TG),INS,fast blood sugar in the group of obesity-related hypertension were significantly higher than those in the normal body mass group(P< 0.01),the values of serum LPL content and high density lipoprotein-C(HDL-C) were the reverse (P< 0.01).The values of total cholesteral(TC)(P> 0.05) and low density lipoprotein-C(LDL-C) (P> 0.05) had no difference. The values of great omentum adipose tissue LPL content in the group of obesity-related hypertension had no significantly difference with control group(P> 0.05);serum LPL content was significantly negatively correlated with BMI(r= -0.640,P< 0.01), WHR(r= -0.589,P< 0.01) and TG(r= -0.784,P< 0.01),but positively correlated with insulin sensitive index (r= 0.533,P< 0.01),HDL-C(r= 0.463,P< 0.01), has no correlated with TC(r= -0.222,P> 0.05),LDL-C(r= -0.213, P> 0.05); great omentum adipose tissue LPL content has no correlation with the above related index(P> 0.05).Conclusion Serum preheparin LPL content is significantly correlated with onset of hypertension and its metabolic disorder; great omentum adipose tissue LPL content has no significant correlation with serum LPL content and metabolic disorder of obesity-related hypertension patients.
